While it\u2019s convenient to categorize people by their disease and its severity, few look in the mirror and see &#8220;a poorly controlled diabetic,&#8221; or &#8220;a terminal cancer patient.&#8221; And who would introduce themselves as a co-morbid patient with multiple complaints? Language evolves and <em>The BMJ<\/em> is seeking to stimulate debate on what is acceptable or unacceptable terminology in today\u2019s world, and amend our house style accordingly. We\u2019ve started by inviting <a href=\"http:\/\/www.bmj.com\/about-bmj\/advisory-panels\/patient-panel-members\">our patient panel<\/a>\u00a0to tell us what words and phrases irk them and suggest possible alternatives. Readers are warmly invited to respond and add to the list.<\/p>\n<ul>\n<li><b>&#8220;Managing&#8221;<\/b><span style=\"font-weight: 400\"> the patient rather than the condition. <strong>&#8220;<\/strong><\/span><b>Bedblocker, frequent flyer, heart sink patient<\/b><span style=\"font-weight: 400\">.<strong>&#8220;<\/strong> <strong>&#8220;<\/strong><\/span><b>Referring, admitting, discharging patients&#8221;\u2014<\/b><span style=\"font-weight: 400\">it comes across as \u201cpatient as a bit in the system\u201d being moved around.<\/span><\/li>\n<li><b>&#8220;Consenting the patient&#8221;\u2014<\/b><span style=\"font-weight: 400\">Let\u2019s change informed consent to <a href=\"http:\/\/journals.sagepub.com\/doi\/full\/10.1177\/0141076813490686\">informed request<\/a>.<\/span><\/li>\n<li><b>&#8220;Being called a client&#8221;\u2014<\/b><span style=\"font-weight: 400\">When my Dad went from being cared for in hospital to being cared at home, he suddenly became &#8220;a client&#8221;<b>\u2014<\/b>a word that irritates the heck out of me not to mention my Dad.<\/span><\/li>\n<li><b>&#8220;Leaving off the term patient&#8221;\u2014<\/b><span style=\"font-weight: 400\">I hear the doctors just say, for example,\u00a0<\/span><b>&#8220;Rheumatoids<\/b><span style=\"font-weight: 400\"><strong>&#8220;<\/strong> when referring to patients with RA\/RD. Doctors who use these words think it&#8217;s fine and seem oblivious to how insulting and rude they sound to people who are patients. In diabetes-related discussions the move is to &#8220;people with diabetes&#8221; or &#8220;PWD.&#8221; So I have been promoting &#8220;people with rheumatoid disease&#8221; (PRD) in our community.<\/span><\/li>\n<li><strong>&#8220;<\/strong><b>A sufferer (or suffering from)&#8221;\u2014<\/b><span style=\"font-weight: 400\">My son (who was harmed in the healthcare system when he was a newborn) finds this term very annoying and he has written a play [which makes this point] called &#8220;<a href=\"https:\/\/vimeo.com\/132170470\">Cal Sheridan, Not Suffering.<\/a>&#8221; It won an award at the Kennedy Center<\/span><span style=\"font-weight: 400\">.<\/span><\/li>\n<li><strong>&#8220;<\/strong><b>Under the care of&#8221;\u2014<\/b><span style=\"font-weight: 400\">I hate this term. I self navigate my way through a wealth of different sub specialists whose views on best management vary. The only person whose care I have been consistently \u201cunder\u201d is myself. <\/span><\/li>\n<li><b>&#8220;Adherence&#8221; and &#8220;failed therapy&#8221; or &#8220;failed treatment&#8221;\u2014<\/b><span style=\"font-weight: 400\">like we somehow failed an exam for not trying hard enough.<\/span><\/li>\n<li><b>&#8220;Subjects&#8221;\u00a0<\/b><span style=\"font-weight: 400\">to describe research participants. This term really irks me.<\/span><\/li>\n<li><b>&#8220;Comply with treatment or be compliant&#8221;\u2014<\/b><span style=\"font-weight: 400\">I suggest flipping it from compliance (to the doctor&#8217;s orders) to <\/span><i><span style=\"font-weight: 400\">achievement <\/span><\/i><span style=\"font-weight: 400\">(of my goal). In the compliance mindset, the patient gets no conceptual credit for having followed the instructions successfully<\/span><\/li>\n<li><strong>&#8220;<\/strong><b>Confined to a wheelchair&#8221;\u2014<\/b><span style=\"font-weight: 400\">That is so wrong. It should be &#8220;freed by a wheelchair&#8221;\u2013 freed to move around independently, freed from exhaustion, freed from being homebound and isolated, freed from broken bones that will plague you forever, free from falls that can kill you, free to add normalcy to your life.<\/span><\/li>\n<li><span style=\"font-weight: 400\">I learnt that there are &#8220;Unworte&#8221; (bad words) that were tainted in blood and dishonoured in German medicine. Words (used as medical terms) used to turn humans into objects of eugenic cleansing. Words that would be harmful to anyone being addressed by them. Hence, I always flinch at the mentioning of <\/span><b>&#8220;selection&#8221;<\/b><span style=\"font-weight: 400\"> processes and <\/span><b>&#8220;euthanasia<\/b><span style=\"font-weight: 400\"><strong>&#8220;<\/strong> and <\/span><b>anyone not giving a patient a name\u2014but a number.<\/b><\/li>\n<li><span style=\"font-weight: 400\"><u>Upsetting and stigmatising language in the HIV\/AIDS world:<\/u> If your viral load is so low it can\u2019t be measured then you are <\/span><b>&#8220;undetectable.&#8221;<\/b><span style=\"font-weight: 400\">\u00a0Everybody loves an undetectable patient. If your viral load is very low, you are termed <strong>&#8220;<\/strong><\/span><b>suppressed.&#8221;<\/b><span style=\"font-weight: 400\">\u00a0It is you, not your viral load. When your treatment stops working then you <strong>&#8220;<\/strong><\/span><b>fail the treatment<\/b><span style=\"font-weight: 400\"><strong>.&#8221;<\/strong> But it&#8217;s the medicine that fails, not the patient. People with hepatitis C who are infected with a strain of Hep C which does not respond to treatment are called <strong>&#8220;<\/strong><\/span><b>non-respondents.&#8221;<\/b><span style=\"font-weight: 400\">\u00a0If accidentally you transmit HIV to your child as a mother, this is termed <strong>&#8220;<\/strong><\/span><b>mother to child transmission.<\/b><span style=\"font-weight: 400\"><strong>&#8220;<\/strong> Vertical transmission is less stigmatising and upsetting.\u00a0<\/span><span style=\"font-weight: 400\">You don&#8217;t even have to have HIV, it is enough if you have sex more or less regularly. Then you are a <strong>&#8220;<\/strong><\/span><b>high-risk individual<\/b><span style=\"font-weight: 400\"><strong>&#8220;<\/strong> or a member of a <strong>&#8220;<\/strong><\/span><b>high-risk group.<\/b><strong>&#8220;<\/strong><\/li>\n<li><b>&#8220;Discharged from the care of X&#8221;\u2014<\/b><span style=\"font-weight: 400\">This term is militaristic. As if care starts and ends with a hospital consultant<b>\u2014<\/b>who mostly you never get to see anyway.<\/span><\/li>\n<li><b>&#8220;Describing children with \u00a0developmental disability and genetic syndromes<\/b><span style=\"font-weight: 400\"><strong>&#8220;<\/strong><b>\u2014<\/b>This is important for it not only reflects, but also informs parents\u2019 and society\u2019s thinking about kids like mine. For example, <strong>&#8220;<\/strong><\/span><b>Defect<\/b><span style=\"font-weight: 400\"><strong>&#8220;<\/strong> or <strong>&#8220;<\/strong><\/span><b>abnormality\/abnormal.<\/b><span style=\"font-weight: 400\"><strong>\u201d\u00a0<\/strong>I prefer anomaly. <strong>&#8220;<\/strong><\/span><b>Mental retardation<\/b><span style=\"font-weight: 400\"><strong>&#8220;<\/strong>\u00a0(mostly everyone prefers intellectual or cognitive disability, or neurodevelopment disability or difference\u2014this is an on-going debate and even the professional organisations have changed their name, but geneticists continue to insist to include this term in their literature.<strong>&#8220;<\/strong><\/span><b>Coarse features<\/b><span style=\"font-weight: 400\"><strong>&#8220;<\/strong> versus distinctive facial features and <strong>&#8220;<\/strong><\/span><b>coarsening of features<\/b><span style=\"font-weight: 400\"><strong>&#8220;<\/strong> when they become more pronounced with age. <strong>&#8220;<\/strong><\/span><b>Healthy&#8221; or &#8220;normal&#8221;<\/b><span style=\"font-weight: 400\">\u00a0to describe someone who does not have a particular syndrome, versus someone who does.<\/span><\/li>\n<li><b>&#8220;Hottentottensch\u00fcrze<\/b><span style=\"font-weight: 400\"><strong>&#8220;<\/strong> This term can (or could) be found in German gynaecology textbooks and medical dictionaries. The Hottentots were an African people almost destroyed by German colonialists. The racial profiling included the myth that the Hottentots could store fat in their bellies \u00a0(like a camel&#8217;s hump). When the women lost weight, their lose skin would flap down over the groin.
