{"id":37460,"date":"2016-09-26T13:45:53","date_gmt":"2016-09-26T12:45:53","guid":{"rendered":"https:\/\/blogs.bmj.com\/bmj\/?p=37460"},"modified":"2016-09-26T13:45:53","modified_gmt":"2016-09-26T12:45:53","slug":"paul-wicks-googles-deepmind-health-group-invites-patient-participation","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/bmj\/2016\/09\/26\/paul-wicks-googles-deepmind-health-group-invites-patient-participation\/","title":{"rendered":"Paul Wicks: Google\u2019s Deepmind health group invites patient participation\u00a0"},"content":{"rendered":"<p><a href=\"https:\/\/blogs.bmj.com\/bmj\/files\/2014\/03\/paul_wicks.jpg\"><img loading=\"lazy\" decoding=\"async\" class=\"alignleft size-full wp-image-31231\" src=\"https:\/\/blogs.bmj.com\/bmj\/files\/2014\/03\/paul_wicks.jpg\" alt=\"paul_wicks\" width=\"160\" height=\"140\" \/><\/a>In the aftermath of <a href=\"https:\/\/www.newscientist.com\/article\/2088056-did-googles-nhs-patient-data-deal-need-ethical-approval\/\">some<\/a> <a href=\"https:\/\/medconfidential.org\/2016\/google-deepmind-part-1\/\">difficult<\/a> questions posed by privacy advocates around the Royal Free\u2019s pilot of Deepmind\u2019s Streams app, this week Deepmind Health invited over 120 patients, patient advocates, carers, and health researchers to a half-day event at Google Headquarters in London.<\/p>\n<p>For the past decade the search giant Google has been able to direct resources from its advertising revenues to a range of innovative projects from <a href=\"http:\/\/www.newyorker.com\/magazine\/2013\/11\/25\/auto-correct\">self-driving cars<\/a> to <a href=\"http:\/\/www.nytimes.com\/2015\/02\/05\/style\/why-google-glass-broke.html\">Google Glass<\/a>, and with <a href=\"https:\/\/www.theguardian.com\/technology\/2016\/jan\/26\/google-expected-to-reveal-growth-of-offshore-cash-funds-to-43bn\">$43bn in cash reserves<\/a> is able to invest deeply in strategic areas. One such investment has been its 2014 acquisition of Deepmind, a British company based in London\u2019s King\u2019s Cross which specializes in machine learning, also known by the trendier, but less accurate moniker of \u201cartificial intelligence.\u201d In February 2016 the company <a href=\"https:\/\/www.youtube.com\/watch?v=CEvMXuAw5DY\">announced the launch<\/a> of \u201cDeepmind Health\u201d which would incorporate vast amounts of electronic health data into machine-learning algorithms that would better prioritise and triage patients at risk of acute kidney injury (AKI) in hospital.<!--more--><\/p>\n<p>At the event (<a href=\"https:\/\/www.youtube.com\/watch?v=wBfBiD38x34\">live-streamed<\/a> for free), Deepmind co-founder Mustafa Suleyman gave more details of his unconventional background for a computing research company; he helped set up a student Muslim Youth Helpline and co-founded a conflict resolution service Reos Partners before joining Deepmind. He emphasized that the group\u2019s aim is to take the potential benefits of the advanced machine learning technology on offer to health applications for the benefits of society. As an example he cited <a href=\"http:\/\/www.bloomberg.com\/news\/articles\/2016-07-19\/google-cuts-its-giant-electricity-bill-with-deepmind-powered-ai\">Deepmind\u2019s 15% improvement to the power usage efficiency<\/a> of Google\u2019s data center, although details were not provided on how this was achieved.<\/p>\n<p>During the meeting a number of patients and clinicians were invited to share their experiences of conditions in which Deepmind Health is prototyping solutions, such as acute kidney injury (AKI) and wet age-related macular degeneration (AMD). While the potential for benefit clearly exists and could be a positive force for good, it is important to clarify that the presentation of information was limited to photos of actors looking at phones, not real data. While individual stories serve as a powerful reminder of the importance of good care, the patient speakers owed their own outcomes to existing medical technology. Questions in the audience from patient advocates underscored the importance of developing the Streams platform securely and to ensure that patients and caregivers should be able to access and add to their medical data; Suleyman agreed and committed to a long-term vision of a patient portal that would incorporate such features.<\/p>\n<p>Invited speaker, <a href=\"http:\/\/data4health.co.uk\/\">Data4Health<\/a> advocate, and cancer survivor Graham Swift evangelized his enthusiasm for sharing of medical data and downplayed the fear of privacy advocates \u2013 he was concerned that health data was being underutilized and sought to \u201cremove the subject of misuse from the top of the agenda and put it in a more sensible place.\u201d His perspective on this year\u2019s reaction to the Deepmind AKI pilot was that if \u201c<a href=\"http:\/\/www.thetimes.co.uk\/article\/google-given-patient-notes-to-help-detect-fatal-illness-jx3xtwm2c\">15,000 people could be saved every year<\/a>\u2026\u201d he was perplexed as to why \u201ca doctor is worried about crossing the I\u2019s and dotting the T\u2019s for 12 months (of regulatory approval)\u201d. While this was an impassioned argument from a health campaigner, a lot of weight rests on that initial \u201cif\u201d, and the burden of proof lies with Deepmind at the beginning, middle, and end of their work to validate the potential of their system continuously and openly.<\/p>\n<p>In further audience discussion, general practitioner and <a href=\"https:\/\/www.patientopinion.org.uk\/\">Patient Opinion<\/a> founder Paul Hodgkin pointed out that any algorithm contains within it value judgments that could be locked away in a \u201cblack box\u201d of an algorithm. In Deepmind\u2019s <a href=\"http:\/\/www.nature.com\/news\/google-ai-algorithm-masters-ancient-game-of-go-1.19234\">celebrated work in beating games like Go<\/a> the way to \u201cwin\u201d was always made explicit, and Hodgkin called upon Suleyman to make the ends which the algorithms were trying to achieve fully transparent \u201cso that we can all see what your algorithms were being trained to do.\u201d On this point Suleyman acknowledged that this was a challenge that needed more work and pointed to their body of published scientific work as an example of their moves towards openness.<\/p>\n<p>One challenge touched upon was that Deepmind Health had not yet established its business model for engaging in healthcare with the NHS. While in the current period such activities were being embarked upon as research endeavors or perhaps even the philanthropic activities of a corporation at its peak, references to \u201csustainability\u201d suggested that at some point a business model must be found. A risk to Deepmind\u2019s ongoing operations will be that without a clear understanding of their side of the social contract, patients (and data guardians) may be hesitant to permit <em>carte blanche<\/em> access that could come with a price later on.<\/p>\n<p>While questions from the audience were polite in a typically British fashion, commenters online such as law and technology reporter Julia Powles (<a href=\"https:\/\/twitter.com\/juliapowles\">@juliapowles<\/a>) posed tougher questions on Twitter; \u201cWhat\u2019s in it for Google?\u201d Research ethicist Jen Pearson (<a href=\"https:\/\/twitter.com\/TheABB\">@TheABB<\/a>) asked why Deepmind could assume that its use of patient data is \u201ca given\u201d when the event was prompted in part by a reaction against their use of data without permission. Tech blogs such as TechCrunch highlight that although billed as a public event, <a href=\"https:\/\/techcrunch.com\/2016\/09\/20\/deepmind-wants-its-healthcare-ai-to-charge-by-results-but-first-it-needs-your-data\/\">the audience were themselves somewhat curated<\/a>. The key asset that Deepmind <a href=\"http:\/\/www.bmj.com\/content\/354\/bmj.i3636\">needs to build now is trust<\/a>, particularly after the care.data debacle in the UK.
