{"id":37440,"date":"2016-09-21T16:55:30","date_gmt":"2016-09-21T15:55:30","guid":{"rendered":"https:\/\/blogs.bmj.com\/bmj\/?p=37440"},"modified":"2017-03-17T12:28:52","modified_gmt":"2017-03-17T11:28:52","slug":"william-cayley-what-is-your-story","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/bmj\/2016\/09\/21\/william-cayley-what-is-your-story\/","title":{"rendered":"William Cayley: What is your story?"},"content":{"rendered":"<p><a href=\"https:\/\/blogs.bmj.com\/bmj\/files\/2014\/07\/bill_cayley_2.jpg\"><img loading=\"lazy\" decoding=\"async\" class=\"alignleft wp-image-31912\" src=\"https:\/\/blogs.bmj.com\/bmj\/files\/2014\/07\/bill_cayley_2-243x300.jpg\" alt=\"bill_cayley_2\" width=\"115\" height=\"141\" srcset=\"https:\/\/blogs.bmj.com\/bmj\/files\/2014\/07\/bill_cayley_2-243x300.jpg 243w, https:\/\/blogs.bmj.com\/bmj\/files\/2014\/07\/bill_cayley_2.jpg 551w\" sizes=\"auto, (max-width: 115px) 100vw, 115px\" \/><\/a>Much has been written in recent years about \u201c<a href=\"http:\/\/jama.jamanetwork.com\/article.aspx?articleid=194300\">narrative medicine<\/a>\u201d\u00a0or \u201c<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3034473\/\">narrative based medicine<\/a>,\u201d\u00a0and there has even been discussion of how to integrate \u201cnarrative\u201d and \u201cevidence based\u201d medicine in both <a href=\"http:\/\/www.thelancet.com\/journals\/lancet\/article\/PIIS0140-6736(08)60156-7\/fulltext\">journal articles<\/a>\u00a0and <a href=\"http:\/\/www.stfm.org\/FamilyMedicine\/Vol46Issue10\/Talen806\">books<\/a>.<\/p>\n<p>Most of this work (very helpfully) focuses on the narratives of patients: who they are as people, how their sufferings affect them, how they create or find meaning, etc. These are all important considerations, but what about our narratives of our work as physicians or healthcare professionals? In what story context do we see our work?<!--more--><\/p>\n<p>Are we \u201chealthcare providers?\u201d The term \u201c<a href=\"http:\/\/jama.jamanetwork.com\/article.aspx?articleid=2506307\">first appeared in the modern healthcare lexicon as a shorthand referring to delivery entities such as group practices, hospitals, and networks<\/a>,\u201d\u00a0and frames the work of the physician primarily in terms of goods, services, and reimbursement. Are we primarily about a commercial exchange over services \u201cprovided?\u201d<\/p>\n<p>Or are we population managers? There has been increasing emphasis on \u201cpop health\u201d (as it is sometimes abbreviated), to the extent that guideline based registries are used to evaluate the work of physicians and to guide (or berate?) patients into \u201ccompliance.\u201d Are we primarily about making sure our patients fit the measures and stay with the program?<\/p>\n<p>Or are we instead <a href=\"https:\/\/blogs.bmj.com\/bmj\/2016\/09\/16\/william-cayley-systems-wisdom\/\">system<\/a>\u00a0managers? Many advances in healthcare (especially around issues of safety) have been inspired by processes used in the airline industry\u2014and often with good effect. However, physicians are not pilots, and people are not planes. Safety in medicine is of the utmost importance, but individual people need to be guided and supported in making their individual decisions\u2014one size does not necessarily fit all.<\/p>\n<p>Ten years ago <a href=\"http:\/\/www.stfm.org\/Portals\/49\/Documents\/FMPDF\/FamilyMedicineVol38Issue1Cayley60.pdf\">I reviewed a book<\/a>\u00a0on the future of primary care, which noted even then that with the current trends in healthcare, patients are often left wondering \u201cwho is my doctor?\u201d An important question at the time, and just as important now.<\/p>\n<p>There are many competing narratives that can frame our work as physicians.
