The Latin root <em>patiens<\/em> (\u201che who suffers\u201d) suggests passivity, particularly when paired with <em>doctore<\/em> (\u201che who teaches\u201d). \u00adSmall wonder, then, that <a href=\"https:\/\/jhupbooks.press.jhu.edu\/content\/silent-world-doctor-and-patient\">physicians have traditionally viewed patient-centeredness<\/a> as their providing &#8220;caring custody&#8221; while acting as \u201crational agents\u201d on patients&#8217; behalf. (1)<!--more--><\/p>\n<p>Seeking to shake up these old assumptions, we clumsily ask providers to create a person- and family-\/caregiver-centric relationship, with everyone becoming &#8220;<a href=\"http:\/\/qualitysafety.bmj.com\/content\/early\/2015\/09\/16\/bmjqs-2015-004315.full\">co-producers<\/a>&#8221; of care. (2) Unfortunately, this mish-mash only muddies what should be clear. What patient or doctor, after all, would describe themselves as \u201cco-producers\u201d of chemotherapy?<\/p>\n<p>More importantly, the awkward terminology deflects a focus on the core of the clinician-patient relationship. Etymology notwithstanding, \u201cpatient\u201d signifies a personal connection deeply rooted in an ethical framework. At a time when those who are ill can be pigeonholed as \u201cconsumers\u201d whose lives comprise but a few bytes of Big Data,<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4061351\/\"> it\u2019s time to reclaim \u201cpatient\u201d and redefine<\/a> that term as representing a rich, collaborative relationship. (3)<\/p>\n<p>The history of two other words suggests success is possible. The first is \u201cqueer,\u201d meant to label homosexuals negatively. The gay rights movement not only turned \u201cqueer studies\u201d into an expression of pride, it also made the word more mainstream.<\/p>\n<p>The popular early-2000s TV show <a href=\"https:\/\/en.wikipedia.org\/wiki\/Queer_Eye\"><em>Queer Eye for the Straight Guy<\/em><\/a> was built upon a premise similar to what happens when a hospital establishes a patient and family advisory council. Apprehensive members of the dominant culture discover that someone whose counsel they never imagined seeking actually gives them highly useful advice. Both surprised and grateful, they realize the \u201cother\u201d isn\u2019t so \u201cother,\u201d after all.<\/p>\n<p>Then there\u2019s \u201cgirl,\u201d often used to infantilize women. In the early 1980s, the word was reclaimed by feminists in <a href=\"https:\/\/en.wikipedia.org\/wiki\/Girls_Just_Want_to_Have_Fun#cite_note-She.27s_a_rebel-6\">Cyndi Lauper\u2019s<\/a> <em>Girls Just Want to Have Fun<\/em>, whose winking lyrics and music video were proclaimed an \u201canthem of female solidarity.\u201d(4) Three decades later, Lena Dunham deliberately titled her hit HBO series about her millennial peers, <a href=\"http:\/\/www.hbo.com\/girls\"><em>Girls<\/em><\/a>.<\/p>\n<p><iframe loading=\"lazy\" title=\"Cyndi Lauper - Girls Just Want To Have Fun (Official Video)\" width=\"640\" height=\"480\" src=\"https:\/\/www.youtube.com\/embed\/PIb6AZdTr-A?feature=oembed\" frameborder=\"0\" allow=\"accelerometer; autoplay; clipboard-write; encrypted-media; gyroscope; picture-in-picture; web-share\" referrerpolicy=\"strict-origin-when-cross-origin\" allowfullscreen><\/iframe><\/p>\n<p>As someone who\u2019s spent years trying to make medical care more patient-centered, I understand that changing clinician behaviour is difficult and often frustrating work. All the more reason to jettison jargon and speak, instead, to the heart. As <a href=\"https:\/\/books.google.co.uk\/books?hl=en&amp;lr=&amp;id=bmOjws1KRBoC&amp;oi=fnd&amp;pg=PA9&amp;dq=Cassell+EJ.+The+healer%27s+art.+Cambridge,+MA:+MIT+Press%3B+1985.&amp;ots=C64b_nRqOF&amp;sig=GVNNaAxkec_2X2p_DbnlBR-AUpE#v=onepage&amp;q&amp;f=false\">Eric Cassell<\/a> wrote in <em>The Healer\u2019s Art<\/em>, \u201cMedicine must be recognized as a moral profession\u2026concerned with the care of persons <em>by<\/em> persons, as simple as that.\u201d (5) That type of relationship by its very nature must involve mutual respect, with no need to specifically list respect for family, friends, \u201cvalues and preferences,\u201d etc.<\/p>\n<p>Moreover, while clinicians certainly should understand a patient\u2019s economic and social circumstances, blithely substituting \u201cconsumer\u201d for \u201cpatient\u201d risks undermining fundamental distinctions between the two terms. Must the patient lying in her hospital bed now keep in mind <em>caveat emptor<\/em> as well as <em>primum non nocere<\/em>?<\/p>\n<p>The strong ethical (and legal) expectations surrounding \u201cpatient\u201d are particularly significant at a time when <a href=\"http:\/\/www.modernhealthcare.com\/article\/20151112\/NEWS\/151119993\">digitized health care data is being hailed<\/a> as \u201cthe new money\u201d (6) and a <a href=\"http:\/\/www.npr.org\/sections\/health-shots\/2016\/07\/30\/487778779\/how-your-health-data-lead-a-not-so-secret-life-online.\">Google search<\/a> can reveal someone\u2019s health status (7). While we patients are no longer passive, nonetheless we remain <em>your<\/em> patients (not \u201ccustomers\u201d) even if your paycheck comes from a large organization. It is you \u2013 doctor, nurse and other care provider \u2013 whom we trust to not exploit our vulnerability.<\/p>\n<p>Feminists and gays refused to be defined by the pejorative intent of \u201cgirls\u201d and \u201cqueers.\u201d Patients can do the same.
