His comments were immediately reported as a &#8220;bed blocking&#8221; crisis.<\/p>\n<p>The term bed blocking certainly trips off the tongue more easily than &#8220;delayed transfers of care,&#8221; but is the term derogatory? I think it is, implying that one patient is somehow denying another access to care.<!--more--><\/p>\n<p>I&#8217;m with medical tweeter Gordon Caldwell and others who responded to him in February:<\/p>\n<blockquote class=\"twitter-tweet\" data-width=\"550\" data-dnt=\"true\">\n<p lang=\"en\" dir=\"ltr\">&quot;Bed Blocker&quot; is an offensive term that depersonalises making it easier to disrespect person &amp; treat without dignity<a href=\"https:\/\/t.co\/XdoLzUVoQA\">https:\/\/t.co\/XdoLzUVoQA<\/a><\/p>\n<p>&mdash; Dr Gordon Caldwell (@doctorcaldwell) <a href=\"https:\/\/twitter.com\/doctorcaldwell\/status\/696364768857198592?ref_src=twsrc%5Etfw\">February 7, 2016<\/a><\/p><\/blockquote>\n<p><script async src=\"https:\/\/platform.twitter.com\/widgets.js\" charset=\"utf-8\"><\/script><\/p>\n<p>&nbsp;<\/p>\n<blockquote class=\"twitter-tweet\" data-width=\"550\" data-dnt=\"true\">\n<p lang=\"en\" dir=\"ltr\">@peterbhrsmityh <a href=\"https:\/\/twitter.com\/doctorcaldwell?ref_src=twsrc%5Etfw\">@doctorcaldwell<\/a> Absolutely agree. People at their most vulnerable should not be referred to in this way.<\/p>\n<p>&mdash; Caroline  Scates (@CarolineScates) <a href=\"https:\/\/twitter.com\/CarolineScates\/status\/696415633442017281?ref_src=twsrc%5Etfw\">February 7, 2016<\/a><\/p><\/blockquote>\n<p><script async src=\"https:\/\/platform.twitter.com\/widgets.js\" charset=\"utf-8\"><\/script><\/p>\n<p>Opposition to the term &#8220;bed blocker&#8221; is not new. In 2000 Christine Hancock, then head of the UK Royal College of Nursing, <a href=\"http:\/\/search.proquest.com\/openview\/d7bbbf29ffc205fb120f5033ce333ad9\/1?pq-origsite=gscholar&amp;cbl=30130\">argued<\/a> that the term implied &#8220;blame and nuisance value,&#8221; but that any blame &#8220;is with the NHS, not the person in the bed.&#8221;<\/p>\n<p>Do patients feel guilty if their hospital stay is extended because of service pressures elsewhere? In February this year MS patient <a href=\"http:\/\/www.mirror.co.uk\/news\/uk-news\/bed-blocking-patients-powerful-letter-7319079\">Poppy Hasted<\/a> wrote an open letter to UK prime minister David Cameron in response to <a href=\"https:\/\/www.gov.uk\/government\/news\/review-shows-how-nhs-hospitals-can-save-money-and-improve-care\">Lord Carter&#8217;s<\/a> 18 month efficiency review.<\/p>\n<p>Carter called for action to be taken on delayed transfers of care, which affects hospitals and trusts\u2019 earning and spending capacity.<\/p>\n<p>Ms Hasted told Mr Cameron in response to the review&#8217;s publication that her three week hospital stay was extended by a month because social services could not organise appropriate home care.<\/p>\n<p>&#8220;I needed three visits each day, with two carers each time\u2014a total of 31.5 hours every week\u2014but social services found it nigh-on impossible to arrange for an agency to provide the carers. So I stayed in hospital and, although it wasn\u2019t my fault, I felt guilty about being there.<\/p>\n<p>&#8220;I knew I was taking up a bed I didn\u2019t need any more and there was someone else lying on a trolley downstairs in A&amp;E, or at home waiting for an operation, who needed it a lot more than I did.&#8221;<\/p>\n<p>In the same month, according to a <em><a href=\"http:\/\/www.telegraph.co.uk\/news\/health\/12160329\/Elderly-hospital-patients-given-eviction-warnings.html\">Daily Telegraph<\/a><\/em> article, hospitals in the West Midlands and Northamptonshire were warning older patients who need residential care that they could face legal proceedings if they are not out within three weeks of being declared medically fit.<\/p>\n<p>So if not bed blocking, what to replace it with? It&#8217;s a tough question, but the ideal term would be one that doesn&#8217;t label vulnerable patients with something negative. It certainly needs to be depersonalised. Delayed transfer? How about an acronym? PAT\u2014Patient Awaiting Transfer? Answers on a postcard please.<\/p>\n<p>Also, is it a politically imposed phrase, as Peter Smith suggested in the above tweet?\u00a0It is hard to discover who first coined the term, but certainly it was in use by 1986\u2014its first appearance in <em>The BMJ. <\/em>At the time <a href=\"https:\/\/en.wikipedia.org\/wiki\/Norman_Fowler\">Norman Fowler<\/a> was health secretary in the Conservative government, and\u00a0<a href=\"https:\/\/en.wikipedia.org\/wiki\/Kenneth_Clarke\">Ken Clarke<\/a>\u00a0had yet to succeed him and introduce the controversial <a href=\"https:\/\/en.wikipedia.org\/wiki\/History_of_the_National_Health_Service_(England)#Thatcher_government_reforms\">purchaser-provider split<\/a>. Might they or their ministerial team have coined the term?<\/p>\n<p>Tellingly, the 1986 research paper <a href=\"http:\/\/www.bmj.com\/content\/bmj\/292\/6530\/1253.full.pdf\">Bed blocking in Bromley<\/a> mentioned bed blocking&#8217;s effect on staff: &#8220;Nurses in one acute medical ward complained of having to be constantly vigilant to prevent ambulant demented patients from wandering and getting lost.
