This, with the addition of a single word, \u201cunintended\u201d, is the definition that <a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/10768430\">Robin Ferner and I<\/a> suggested 16 years ago. Here I gloss some terms in the definition.<\/p>\n<p>\u2022 \u201c<em>Failure<\/em>\u201d signifies that the process has fallen below some attainable benchmark and is the criterion whereby an error is recognised. This derives from <a href=\"http:\/\/www.goodreads.com\/review\/show\/825585536?book_show_action=true&amp;from_review_page=1\">James Reason\u2019s<\/a> definition of an error<!--more-->: \u201cThe failure of planned actions to achieve their desired ends without the intervention of some unforeseeable event.\u201d<\/p>\n<p>\u2022 The \u201c<em>treatment process<\/em>\u201d includes manufacture, regulatory controls, commercialisation (labelling, packaging, marketing information), prescribing, transcribing, dispensing, and administration of a drug, and subsequent monitoring and adjustment of therapy in treating symptoms or their causes or investigating or preventing disease or physiological changes. It encompasses not only therapeutic drugs but also oral contraceptives, vaccines, hormones for replacement therapy, radiographic contrast media, and cells.<\/p>\n<p>\u2022 The phrase \u201c<em>has the potential<\/em>\u201d implies that whether or not an error causes harm, it should be logged, for education and future reference, since it may point to measures designed to prevent further similar errors, which may be harmful. This also reminds us of the final clause in Reason\u2019s definition, recognising that errors are often intercepted. For example, a prescriber may write a prescription for \u201cdigoxin 250 mg\u201d, but whoever is responsible for administering it may recognise the error and correctly administer 250 micrograms instead, preferably after checking with the prescriber.<\/p>\n<p>\u2022 \u201c<em>Unintended<\/em>\u201d is related to the definition of error in the <em>Oxford English Dictionary<\/em>: \u201csomething incorrectly done through ignorance or inadvertence\u201d. If the failure was intended, that would constitute a violation, a subject for another time.<\/p>\n<p>James Reason classified errors into two broad categories, based on the observation that intentions are carried out according to formulated plans. Either the plan is appropriate but carried out wrongly, resulting in errors that he called slips and lapses, or the plan is inappropriate, leading to errors that he called mistakes. This is potentially confusing, since in common parlance \u201cerror\u201d and \u201cmistake\u201d are regarded as synonyms. A more useful approach is Reason\u2019s classification according to whether the error is based on erroneous <u>m<\/u>emory, <u>a<\/u>ctions, <u>r<\/u>ules, or <u>k<\/u>nowledge (MARK), as illustrated in the figure.<\/p>\n<figure id=\"attachment_36805\" aria-describedby=\"caption-attachment-36805\" style=\"width: 455px\" class=\"wp-caption alignnone\"><img loading=\"lazy\" decoding=\"async\" class=\"wp-image-36805\" src=\"https:\/\/blogs.bmj.com\/bmj\/files\/2016\/06\/aronson_marking_2.png\" alt=\"aronson_marking_2\" width=\"455\" height=\"298\" srcset=\"https:\/\/blogs.bmj.com\/bmj\/files\/2016\/06\/aronson_marking_2.png 491w, https:\/\/blogs.bmj.com\/bmj\/files\/2016\/06\/aronson_marking_2-300x197.png 300w\" sizes=\"auto, (max-width: 455px) 100vw, 455px\" \/><figcaption id=\"caption-attachment-36805\" class=\"wp-caption-text\">A classification of medication errors, based on a paradigm of James Reason<\/figcaption><\/figure>\n<p>This classification leads to ideas about how medication errors may be prevented, as summarised in the table below. The percentages are taken from <a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/26257248\">a study of methotrexate<\/a>.<\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"alignnone wp-image-36804 size-full\" src=\"https:\/\/blogs.bmj.com\/bmj\/files\/2016\/06\/aronson_marking.png\" alt=\"aronson_marking\" width=\"481\" height=\"134\" srcset=\"https:\/\/blogs.bmj.com\/bmj\/files\/2016\/06\/aronson_marking.png 481w, https:\/\/blogs.bmj.com\/bmj\/files\/2016\/06\/aronson_marking-300x84.png 300w\" sizes=\"auto, (max-width: 481px) 100vw, 481px\" \/><\/p>\n<p>It is fortunate that memory based errors are uncommon, because memory is hard to improve. But an important principle is suggested by the origin of the word \u201cmemory\u201d, the Indo-European root [S]MER, which implied being mindful or taking care. In Greek \u03bc\u03ad\u03c1\u03bc\u03b5\u03c1\u03bf\u03c2 meant fastidious, and memor in Latin meant mindful. To commemorate is to remember someone or something, perhaps with a memorial. A martyr is remembered. To mourn is to remember sorrowfully. A moratorium is a postponement to allow mindfulness, in Sanskrit sm\u1e5btyupasth\u0101na.
