A penicillin anniversary"},"content":{"rendered":"<p>Today, 12 February, is the 75<sup>th<\/sup> anniversary of the first clinical use of <a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/1541305\">penicillin<\/a> in Oxford in 1941 (picture).<\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"alignnone wp-image-36180\" src=\"https:\/\/blogs.bmj.com\/bmj\/files\/2016\/02\/aronson_penicillin-300x171.png\" alt=\"aronson_penicillin\" width=\"452\" height=\"258\" srcset=\"https:\/\/blogs.bmj.com\/bmj\/files\/2016\/02\/aronson_penicillin-300x171.png 300w, https:\/\/blogs.bmj.com\/bmj\/files\/2016\/02\/aronson_penicillin.png 591w\" sizes=\"auto, (max-width: 452px) 100vw, 452px\" \/><\/p>\n<p><strong>Image:<\/strong> A plaque commemorating the first administration of purified penicillin to a patient in the Radcliffe Infirmary on 12 February 1941\u00a0by Dr Charles Fletcher; the word \u201csystematic\u201d is not necessarily an error; the word has occasionally, albeit rarely, been used to mean systemic, the word that we would now use, and the <em>OED<\/em> lists examples from the 19<sup>th<\/sup>, 20<sup>th<\/sup>, and even the 21<sup>st<\/sup> centuries.<!--more--><\/p>\n<p>Alexander Fleming invented the word \u201cpenicillin\u201d, which he first used <a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC2048009\/pdf\/brjexppathol00255-0037.pdf\">in an article<\/a> in the <em>British Journal of Experimental Pathology<\/em> in 1929, titled \u201cOn the antibacterial action of cultures of a Penicillium, with special reference to their use in the isolation of B. influenz\u00e6\u201d:<\/p>\n<blockquote><p>\u201cIn the rest of this article allusion will constantly be made to experiments with filtrates of a broth culture of this mould, so for convenience and to avoid the repetition of the rather cumbersome phrase \u2018Mould broth filtrate\u2019, the name \u2018penicillin\u2019 will be used. This will denote the filtrate of a broth culture of the particular penicillium with which we are concerned.\u201d<\/p><\/blockquote>\n<p>\u201cB. influenzae\u201d is what we now call <em>Haemophilus influenzae<\/em>. The \u201cparticular penicillium\u201d to which Fleming referred was <em>Pencillium rubrum<\/em>.<\/p>\n<p><em>Penicillium<\/em> was the name that Heinrich Friedrich Link gave in 1809 to a genus of ascomycetous fungi, in a dissertation titled \u201c<a href=\"http:\/\/wwwuser.gwdg.de\/~subtypo3\/gdz\/pdf\/PPN608227714_0003\/PPN608227714_0003___LOG_0003.pdf\">Observationes in ordines plantarum naturales. Dissertatio I<sup>ma<\/sup> complectens Anandrarum oridiknes Epiphytas, Mucedines Gastromycos et Fungos<\/a>\u201d published in the Magazin der Gesellschaft Naturforschender Freunde zu Berlin. Link listed Penicillium as the tenth genus in the section on Mucedines and described three species, <em>Penicillium candidum<\/em>, <em>expansum<\/em>, and <em>glaucum<\/em>.<\/p>\n<p>The Indo-European root PES meant a tail, as did the Latin word penis. The diminutive penicillus, a little tail, was used to mean a brush, and specifically a paintbrush. This came into Old and Middle French as pincel or peincel (in modern French, pinceau) and then into English as pencil, which originally meant a paintbrush, typically one with fine hair tapered to a point, suitable for delicate work, and later any tapered or pointed instrument for writing or drawing. \u201cPencil\u201d is also slang for the penis, echoing the original etymology. <em>Penicillium <\/em>species have conidiophores and conidia, brush-like asexual fruiting structures (picture).<\/p>\n<figure id=\"attachment_36181\" aria-describedby=\"caption-attachment-36181\" style=\"width: 193px\" class=\"wp-caption alignleft\"><img loading=\"lazy\" decoding=\"async\" class=\"wp-image-36181 \" src=\"https:\/\/blogs.bmj.com\/bmj\/files\/2016\/02\/aronson_penicillin2-223x300.png\" alt=\"aronson_penicillin2\" width=\"193\" height=\"260\" srcset=\"https:\/\/blogs.bmj.com\/bmj\/files\/2016\/02\/aronson_penicillin2-223x300.png 223w, https:\/\/blogs.bmj.com\/bmj\/files\/2016\/02\/aronson_penicillin2.png 224w\" sizes=\"auto, (max-width: 193px) 100vw, 193px\" \/><figcaption id=\"caption-attachment-36181\" class=\"wp-caption-text\">The brush-like coniodophores and conidia of <em>Penicillium<\/em>.<\/figcaption><\/figure>\n<p>Penicillin was not the first antibacterial drug. The sulfonamides, for example, were discovered by <a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/24727964\">Gerhard Domagk<\/a> in 1932. But penicillin was the first antibiotic, defined as a substance that is produced by living organisms and that can destroy or inhibit the growth of micro-organisms. Today we use the term \u201cantibacterial\u201d to refer to antibiotics and other chemotherapeutic agents that are effective against bacteria and \u201cantimicrobial\u201d to broaden the spectrum to include protozoa, viruses, and other micro-organisms. <a href=\"http:\/\/blogs.lshtm.ac.uk\/news\/2013\/09\/19\/the-drugs-dont-work-the-global-threat-of-antimicrobial-resistance\/\">Recent discussions<\/a> about the dangers of the emergence of organisms that are becoming increasingly resistant to antimicrobial drugs may have made it seem that this is a new problem. <a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/18746315\">It is not<\/a>.<\/p>\n<p>&nbsp;<\/p>\n<p>For nearly 40 years I have been associated, as author and editor, with the <em>International Encyclopedia of Adverse Drug Reactions and Interactions<\/em>, more commonly known by its colloquial title <em>Meyler\u2019s Side Effects of Drugs<\/em>, the 16th edition of which has recently been published, and with its annual updates, the <em>Side Effects of Drugs Annuals<\/em> (SEDA). Over 30 years ago, in a chapter on adverse reactions to antibiotics in SEDA-9 (Elsevier, 1985), one of the contributing authors, Tore Midtvedt, wrote about the dangers:<\/p>\n<blockquote><p>\u201cIn order to avoid the further development and spread of resistant agents\u2014a side effect more important than any other\u2014we desperately need to attain (or revert to?) a more intelligent use\u2014in clinical as well as industrial situations\u2014of the antimicrobials available. The writing \u2018you have been weighed in the balance and found wanting\u2019 is already on the wall.\u201d<\/p><\/blockquote>\n<p>As others have, Midtvedt repeatedly wrote about this problem. In SEDA-10, for example (1986): \u201cWe can hardly blame the microbes. The situation is created by our use\u2014and misuse\u2014of the antimicrobial agents available.\u201d<\/p>\n<p>Perhaps at last note is being taken.<\/p>\n<p><a href=\"https:\/\/blogs.bmj.com\/bmj\/files\/2014\/12\/jeffrey_aronson.jpg\"><img loading=\"lazy\" decoding=\"async\" class=\"alignleft wp-image-32935\" src=\"https:\/\/blogs.bmj.com\/bmj\/files\/2014\/12\/jeffrey_aronson-223x300.jpg\" alt=\"jeffrey_aronson\" width=\"123\" height=\"165\" srcset=\"https:\/\/blogs.bmj.com\/bmj\/files\/2014\/12\/jeffrey_aronson-223x300.jpg 223w, https:\/\/blogs.bmj.com\/bmj\/files\/2014\/12\/jeffrey_aronson.jpg 446w\" sizes=\"auto, (max-width: 123px) 100vw, 123px\" \/><\/a><\/p>\n<p>&nbsp;<\/p>\n<p><em><strong>Jeffrey Aronson<\/strong> is a clinical pharmacologist, working in the Centre for Evidence Based Medicine in Oxford&#8217;s Nuffield Department of Primary Care Health Sciences.
