Brinkmanship, perhaps, or fear of something (losing votes?), or perhaps hoping to destroy the system as an excuse <a href=\"http:\/\/www.bmj.com\/content\/351\/bmj.h6279\">to privatize the NHS<\/a>? Then suddenly, and keeping company with the Chancellor of the Exchequer, a U-turn\u2014he agrees.<\/p>\n<p>So, while we wait, here\u2019s clarification about conciliation and reconciliation.<!--more--> The hypothetical Indo-European root KEL meant to shout and hence to summon or instruct. The Roman calends was the day on which the new month was proclaimed, the start of the calendar. Accounts were settled on the calends and recorded on scrolls (cylinders), although \u201ccylinder\u201d probably comes from a different but similar root [S]KEL, curved, as in scolex and scoliosis. \u201cThe Greek calends\u201d was never\u2014Greek months had no calends. Marigolds (<em>Calendula<\/em>) were supposed to blossom on the calends. To intercalate was to insert an extra day or month in the calendar, and hence to interpolate in general. A paraclete (Greek <em>\u03c0\u03b1\u03c1\u03b1\u03ba\u03b1\u03bb\u03b5\u1fd6\u03bd<\/em>, to call for help) is an advocate or intercessor, typically the Holy Spirit, but it could describe patient advocates\u2014paraclinical paracletes. It could also describe Acas, who will hear your claims (metathesizing KEL to KLA) and clarify possible solutions.<\/p>\n<p>Adding con- (Latin cum) to a root such as KEL introduces the idea of with-ness. A council consults and gives counsel, as Acas should do. And adding re- in front of that invokes again-ness, either repeating or restoring something. So re-con-ciliation literally involves repeatedly collaborating in order to clarify.<\/p>\n<p>Reconciliation involves reunion or restoration, with harmony in mind; Mr Hunt might find that helpful to remember. In the 14th century, starting with relations between God and man, it meant the restoration of humanity to God\u2019s favour, especially through Christ&#8217;s sacrifice. Then, extended to relations between man and man, it came to mean restoring estranged people or parties to friendship. It was later used to describe relations between different things as well; for example, resolving apparent discrepancies in bookkeeping accounts.<\/p>\n<p>The latest variety is medicines (or medication) reconciliation. It is defined by the Institute for Healthcare Improvement (IHI) as \u201cthe process of identifying the most accurate list of a patient\u2019s current medicines\u2014including the name, dosage, frequency, and route\u2014and comparing them to the current list in use, recognizing discrepancies, and documenting any changes, thus resulting in a complete list of medications, accurately communicated\u201d. It is part of what has been called \u201c<a href=\"http:\/\/www.bmj.com\/content\/327\/7425\/1219.long\">information continuity<\/a>\u201d. \u201cHarmonization\u201d might be a better term, but the definition is well crafted, although it hasn&#8217;t yet made it into the <em>OED<\/em>.<\/p>\n<p>Medication reconciliation seems to have been first described <a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/14691892\">in 2003<\/a>, with the observation that since many medication errors occur when a patient moves from one place to another, one strategy for reducing the risk of adverse reactions is \u201cto reconcile the medication orders between the two transition points\u201d. The IHI has developed a \u201c<a href=\"http:\/\/www.ihi.org\/resources\/Pages\/Tools\/UniversalMedicationForm.aspx\">Universal Medication Form<\/a>\u201d to help patients maintain their own records, and \u201cmed rec\u201d, as some call it, is one of the five elements in the <a href=\"http:\/\/www.who.int\/patientsafety\/implementation\/solutions\/high5s\/en\">World Health Organization\u2019s \u201cHigh 5s\u201d project<\/a>.<\/p>\n<p>The process has five steps: list the current medications; list the medications currently needed; compare the lists; make a new list based on the comparison; communicate the new list to the patient and caregivers. NHS Education for Scotland offers general practitioners a <a href=\"http:\/\/www.scottishpatientsafetyprogramme.scot.nhs.uk\/programmes\/primary-care\/safer-medicines\">Medicines Reconciliation Care Bundle<\/a>, and UK general practitioners can access a Medicines Reconciliation Tool on the Royal College of General Practitioners&#8217;\u00a0website as part of a <a href=\"http:\/\/www.rcgp.org.uk\/clinical-and-research\/toolkits\/patient-safety.aspx\">Patient Safety Toolkit<\/a>.<\/p>\n<p>Patients who have their own electronic medical records can maintain reconciliated lists of their medications, enabling them to understand them better, and to link them to other information, such as test results.
