{"id":35579,"date":"2015-10-30T17:09:45","date_gmt":"2015-10-30T16:09:45","guid":{"rendered":"https:\/\/blogs.bmj.com\/bmj\/?p=35579"},"modified":"2015-11-02T11:10:41","modified_gmt":"2015-11-02T10:10:41","slug":"a-changing-world-and-what-it-means-for-medical-training","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/bmj\/2015\/10\/30\/a-changing-world-and-what-it-means-for-medical-training\/","title":{"rendered":"Global Health Curriculum group: A changing world and what it means for medical training"},"content":{"rendered":"<p><a href=\"https:\/\/blogs.bmj.com\/bmj\/files\/2015\/10\/core_global_health_competencies.png\"><img loading=\"lazy\" decoding=\"async\" class=\"alignleft size-thumbnail wp-image-35581\" src=\"https:\/\/blogs.bmj.com\/bmj\/files\/2015\/10\/core_global_health_competencies-150x150.png\" alt=\"core_global_health_competencies\" width=\"150\" height=\"150\" \/><\/a>This month the BMA released <a href=\"http:\/\/bma.org.uk\/news-views-analysis\/news\/2015\/october\/bma-defines-core-principles-of-training-for-doctors\">a report<\/a> on the need for pre and post-graduate medical education and training\u00a0to adapt in the face of a rapidly &#8220;changing world.&#8221;<\/p>\n<p>We are pleased to see recognition of the need to update postgraduate competencies. However, as doctors in training who are dedicated to the\u00a0integration of global health into current postgraduate training programmes,\u00a0we fear that there are important gaps that have not been adequately\u00a0addressed. The report recognises increased complexity in &#8220;characteristics of\u00a0the population&#8221; and &#8220;public expectations of healthcare,&#8221; but we believe that\u00a0equally important is the increasing complexity of social, political, and economic\u00a0systems which profoundly influence health.\u00a0<!--more--><\/p>\n<p>We feel that the BMA document does not take a wide enough view of our\u00a0changing world. While it is important to recognise the evolution of behaviours,\u00a0lifestyles, and the diseases that we see and treat, it is also important to\u00a0recognise how the opportunities and challenges to address health issues are\u00a0evolving. Our roles and ability to improve health are determined by issues\u00a0outside the sphere of local population characteristics and expectations.<\/p>\n<p>Globalisation means that the world is ever more interconnected, and every\u00a0health professional is affected by and has an influence on global forces. For\u00a0example, doctors rely on and are involved in global trade systems when they\u00a0prescribe medications or use medical devices. An increasing number of state\u00a0and corporate actors play a part in determining economic determinants of\u00a0health, from the price of drugs (the price of pyrimethamine and cycloserine, covered in <em><a href=\"http:\/\/www.theguardian.com\/business\/2015\/sep\/21\/entrepreneur-defends-raise-price-daraprim-drug\" target=\"_blank\">The Guardian<\/a><\/em> and <em><a href=\"http:\/\/www.nytimes.com\/2015\/09\/21\/business\/a-huge-overnight-increase-in-a-drugs-price-raises-protests.html?_r=0\" target=\"_blank\">New York Times<\/a>,<\/em>\u00a0are recent examples)\u00a0to the wealth or poverty of nations\u00a0and people. Recently, increasing international migration has brought home the\u00a0concept of globalisation and the profound influence of global forces.<\/p>\n<p>Our understanding of health and illness, and\u00a0our\u00a0ability to manage them, is affected by\u00a0multiple overarching factors, including technological advances; governance\u00a0structures; and knowledge advances in the subjects\u00a0of the environment, sociology,\u00a0epidemiology, and systems thinking.<\/p>\n<p>Because the world is changing at an\u00a0unprecendented rate, tomorrow&#8217;s doctors require not only knowledge of the\u00a0current situation, but also the skills to learn about and adapt to future\u00a0scenarios.<\/p>\n<p>The report explores a doctor\u2019s experience of the workplace and career\u00a0pathway. While it goes some way to acknowledging the changing nature of\u00a0health workforces, the report does not explore the implications of health\u00a0worker migration for health professionals today and in the future. Trainees\u00a0need to learn to play a part\u00a0in establishing efficient and effective health\u00a0systems for the benefit of all staff and service users. By engendering an ethos\u00a0of leadership, in a <a href=\"http:\/\/www.mee.nhs.uk\/PDF\/14274%20Bookmark%20Web%20Version.pdf\" target=\"_blank\">culture of learning<\/a>,\u00a0and with an awareness of the wider determinants of health, career satisfaction\u00a0will potentially follow.<\/p>\n<p>As an influential player in the development of doctors\u2019 training, it is essential\u00a0that the BMA sets out a vision that is responsive to the ever changing political,\u00a0social, environmental, and economic context. This context defines the health\u00a0needs of the population and the roles that doctors can have\u00a0in protecting,\u00a0restoring, and promoting health.
