We should treat (in both senses of the word) patients more like people and less like objects we control. We should stop dehumanising patients.<\/p>\n<p>I was challenged to think about how we do just that as I listened to <a href=\"http:\/\/gimletmedia.com\/episode\/2-instagram-for-doctors\/\">this podcast<\/a>\u2014it&#8217;s worth a listen all the way through.<!--more--><\/p>\n<p><iframe loading=\"lazy\" title=\"#2 The Secret, Gruesome Internet For Doctors by Reply All\" width=\"640\" height=\"400\" scrolling=\"no\" frameborder=\"no\" src=\"https:\/\/w.soundcloud.com\/player\/?visual=true&#038;url=https%3A%2F%2Fapi.soundcloud.com%2Ftracks%2F178489631&#038;show_artwork=true&#038;maxheight=960&#038;maxwidth=640\"><\/iframe><\/p>\n<p>It is an episode of <a href=\"http:\/\/gimletmedia.com\/show\/reply-all\/%20\">Reply All<\/a>, a podcast about the internet, which\u00a0looks at the medical image app <a href=\"https:\/\/app.figure1.com\">Figure 1<\/a>. The app allows healthcare professionals to upload\u2014and comment on\u2014clinical images. The existence and popularity of the app raises a lot of questions. How should doctors behave on social media? Is the confidentiality issue covered enough? How do patients feel about doctors talking like this? Is the app educational? (Some of those issues are covered in more depth <a href=\"http:\/\/www.theawl.com\/2014\/10\/the-uncanny-language-of-medical-instagram\">in this article on The Awl<\/a>.<\/p>\n<p>A lot of the podcast is about the difference in perception of these images by doctors and lay people. And one of the first questions the (generally squeamish and appalled) non-medic reporter asks is this: do I find these images more shocking because I am prevented from seeing them as part of a human being?<\/p>\n<p>In medical publishing, and on this app, we use non-identifiable images. It is a legal requirement <a href=\"http:\/\/www.hhs.gov\/ocr\/privacy\/hipaa\/understanding\/index.html\">in the US<\/a> and the UK, and an integral part of <em>The BMJ<\/em>\u2019s <a href=\"http:\/\/www.bmj.com\/about-bmj\/resources-authors\/forms-policies-and-checklists\/use-images\">policy of protecting patient confidentiality<\/a>.<\/p>\n<p>But there\u2019s a question: by protecting patients\u2019 confidentiality, do we dehumanise them?<\/p>\n<p>If bits of a human are not identified as parts of a whole person, what effect does that have on how we perceive people and disease? Maybe the conditioning to see the disease and not the person starts and is perpetuated here.<\/p>\n<p>In the podcast, Lauren (a resident who uses Figure 1) expresses surprise at the reporter saying the images are \u201csome of the most horrific things he\u2019s seen on the internet.\u201d<\/p>\n<p>\u201cMost of it\u2019s not Gawker [<a href=\"http:\/\/gawker.com\/\">a popular gossip blog<\/a>] worthy, but maybe I have lost more empathy than I thought.\u201d<\/p>\n<p>Lauren wonders whether being interested in the disease or the injury processes that caused a patient\u2019s death might seem weird to people who see only tragedy. I know what she means. I often catch myself reading an obituary looking for the punchline . . . how did they die?<\/p>\n<p>I don\u2019t know whether it&#8217;s that particularly gruesome type of curiosity that made me want to be a doctor, or if it&#8217;s a way of thinking I\u2019ve been trained into. In looking for the clinical facts, am I distracting myself from feeling human compassion? Perhaps it\u2019s our\u00a0education that conditions us to objectify patients, but maybe it&#8217;s instinctive self-preservation.<\/p>\n<p>As Lauren says: \u201cYou can\u2019t feel everyone\u2019s pain as if it were your own or else you wouldn\u2019t get through the day.\u201d<\/p>\n<p>I\u2019m inclined to agree, but I don\u2019t think that is necessarily what patients are asking professionals to do.
