{"id":32907,"date":"2014-12-04T15:23:56","date_gmt":"2014-12-04T14:23:56","guid":{"rendered":"https:\/\/blogs.bmj.com\/bmj\/?p=32907"},"modified":"2022-01-13T16:35:09","modified_gmt":"2022-01-13T15:35:09","slug":"carl-heneghan-evidence-based-medicine-on-trial","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/bmj\/2014\/12\/04\/carl-heneghan-evidence-based-medicine-on-trial\/","title":{"rendered":"Carl Heneghan: Evidence based medicine on trial"},"content":{"rendered":"<p><a href=\"https:\/\/blogs.bmj.com\/bmj\/2014\/01\/10\/carl-heneghan-what-could-cause-you-to-absorb-water-better-than-water\/carl_heneghan-2\/\" rel=\"attachment wp-att-30747\"><img loading=\"lazy\" decoding=\"async\" class=\"alignleft size-thumbnail wp-image-30747\" src=\"https:\/\/blogs.bmj.com\/bmj\/files\/2014\/01\/carl_heneghan1-150x150.jpg\" alt=\"carl_heneghan\" width=\"150\" height=\"150\" srcset=\"https:\/\/blogs.bmj.com\/bmj\/files\/2014\/01\/carl_heneghan1-150x150.jpg 150w, https:\/\/blogs.bmj.com\/bmj\/files\/2014\/01\/carl_heneghan1-300x300.jpg 300w, https:\/\/blogs.bmj.com\/bmj\/files\/2014\/01\/carl_heneghan1.jpg 480w\" sizes=\"auto, (max-width: 150px) 100vw, 150px\" \/><\/a>Evidence based medicine (EBM) should form the foundation of effective clinical decision making; however, growing unrest\u2014and an awful lot of criticism\u2014suggests the evidence bit of EBM is increasingly part of the problem, and not the solution.<!--more--><\/p>\n<p>Concerns with quality and rigour in research are leading to a lack of trust in the production, publication, and utilisation of evidence. <a href=\"\/\/www.bmj.com\/content\/348\/bmj.g22\">Des Spence, writing in <em>The BMJ<\/em><\/a>, thinks the situation is so bad that, &#8220;Evidence based medicine is broken,&#8221; and when an official from the US Food and Drug Administration (FDA)\u00a0also reports, &#8220;<a href=\"http:\/\/www.bmj.com\/content\/347\/bmj.f6980\">The clinical trial system is broken and it\u2019s getting worse<\/a>,&#8221; you have to acknowledge that there might be problems with the evidence base.<\/p>\n<p>What exactly are these problems?\u00a0In the broadest context, they can be categorised into three main areas\u2014although this doesn\u2019t do justice to the extent of the issues\u2014these are: distortion of the research agenda, very poor quality research, and a lack of transparency for published evidence.<\/p>\n<p>Distortion of the research agenda, mainly by commercial decisions, is leading to an ever increasing evidence base that doesn\u2019t meet the needs of patients. With too much focus on the well, and not enough on the sick, the paradoxical situation has arisen whereby medicine is potentially harming the healthy through <a href=\"http:\/\/www.bmj.com\/content\/344\/bmj.e3502\">earlier detection and increasingly looser definitions of disease<\/a>.<\/p>\n<p>As an example, if you find yourself constantly late, disorganised, forgetful, and overwhelmed by your responsibilities\u2014which could refer to all of us\u2014you might have adult attention-deficit disorder. You will be pleased to know that\u00a0there are at least four different medications currently available for this condition. You could argue that the pharmaceutical industry is becoming equally good, if not better, <a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC1369125\/\">at manufacturing diseases as opposed to drugs<\/a>.<\/p>\n<p>While research publications continue to increase\u2014to\u00a0the point where the notion of keeping up to date is nigh on impossible\u2014the quality is often very poor, if not sometimes outright pitiful. Although there has been a growth in research to promote implementation, my observations\u2014note the low level of my evidence \u2014have been that while clinicians are extremely good at responding to robust evidence, all too often the quality of the evidence is weak and unworthy of implementation.<\/p>\n<p>Poor quality evidence arises when observational data\u00a0are used to establish treatment effects; when outcome\u00a0measures are unimportant to patients or, even worse, are meaningless for patient care; and also when simple factors to account for bias are not incorporated into the research design.<\/p>\n<p>John Ioannidis, in his highly cited publication \u201c<a href=\"http:\/\/www.plosmedicine.org\/article\/info%3Adoi%2F10.1371%2Fjournal.pmed.0020124\">Why Most Published Research Findings Are False<\/a>,\u201d cites six helpful pointers\u2014worth committing to memory\u2014to determine if research findings are less likely to be true: studies which are small in size; studies with small effect sizes; hypothesis generating experiments; studies with greater flexibility in design, definitions, and outcomes; investigators with conflicts of interests; and lastly, the hotter a scientific field is the less likely it is to be true.<\/p>\n<p>Finally, incomplete reporting, misrepresentation (better known as spin), publication bias, and falsification of data are all presenting huge problems. As examples, Retraction Watch recently reported that <em>BioMed Central<\/em> &#8220;<a href=\"http:\/\/retractionwatch.com\/2014\/11\/25\/publisher-discovers-50-papers-accepted-based-on-fake-peer-reviews\/\">has uncovered about 50 manuscripts in their editorial system that involved fake peer reviewers<\/a>.&#8221;\u00a0It seems that authors providing their own peer reviewers is now off the table. Further to this, half of all medical reporting is subject to spin, based on abstracts, <a href=\"\/\/www.nhs.uk\/news\/2012\/09September\/Pages\/Half-of-all-medical-reporting-is-subject-to-spin.aspx\">which lead to \u201csexed up\u201d press releases<\/a>. What is more concerning\u2014and led to the <a href=\"http:\/\/www.alltrials.net\/\">AllTrials campaign<\/a>\u2014is that up to half of all clinical trials are never published. Along with reporting bias, and difficulties with data access, the published research as it stands is a barrier to innovation.<\/p>\n<p>An evidence based approach for clinical practice, hence, involves being aware of the evidence, its strengths and weakness, the substantial limitations, and the inferences we subsequently make to inform clinical decision making. Moreover, realising decisions are based not only on evidence is important to understand how to use it, despite all of its limitations, in practice. This<a href=\"http:\/\/www.bmj.com\/content\/348\/bmj.g3725.long\"> point is made by Trisha Greenhalgh and colleagues<\/a>,\u00a0when they say, &#8220;research evidence may still be key to making the right decision\u2014but it does not determine that decision,&#8221; and <a href=\"http:\/\/www.medicossinmarca.cl\/wp-content\/uploads\/2012\/07\/Corruption-of-the-Evidence-as-Threat-and-Opportunity-for-Evidence-Based-Medicine.pdf\">similarly by Gordon Guyatt, and Victor Montori<\/a>, &#8220;the evidence alone never tells one what to do.&#8221;<\/p>\n<p>Distillation of the solution includes first recognising that we have a problem. Clinicians require skills to interpret and evaluate the evidence. Errors in interpretation prevent effective implementation, and, in some situations, will be dangerous for patients.<\/p>\n<p>If the solution isn\u2019t to be EBM, then I look forward to the alternative suggestions and the jury\u2019s decision at <a href=\"https:\/\/ebmlive.org\/\">Evidence Live<\/a> (13-14 April 2015).
