{"id":32144,"date":"2014-08-13T10:52:11","date_gmt":"2014-08-13T09:52:11","guid":{"rendered":"https:\/\/blogs.bmj.com\/bmj\/?p=32144"},"modified":"2014-08-13T10:52:11","modified_gmt":"2014-08-13T09:52:11","slug":"the-bmj-today-doctors-in-scotland-plan-to-vote-against-independence","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/bmj\/2014\/08\/13\/the-bmj-today-doctors-in-scotland-plan-to-vote-against-independence\/","title":{"rendered":"The BMJ Today: Doctors in Scotland plan to vote against independence"},"content":{"rendered":"<p><a href=\"https:\/\/blogs.bmj.com\/bmj\/2012\/04\/30\/zosia-kmietowicz-leaping-out-of-inequalities-the-power-of-imaginative-play\/zosiak\/\" rel=\"attachment wp-att-16707\"><img loading=\"lazy\" decoding=\"async\" class=\"alignleft size-full wp-image-16707\" src=\"https:\/\/blogs.bmj.com\/bmj\/files\/2012\/04\/zosiak.jpg\" alt=\"\" width=\"160\" height=\"120\" \/><\/a>The results of a survey <a href=\"http:\/\/www.bmj.com\/content\/349\/bmj.g5072\">published today by <em>The BMJ<\/em><\/a> on how doctors in Scotland plan to vote in the referendum next month found that 60% plan to vote \u201cno\u201d to independence, while just a third (33%) said they intend to vote \u201cyes.\u201d Doctors are more staunchly opposed to independence than the overall population of Scotland, which puts the \u201cyes\u201d vote at 37%, the \u201cno\u201d vote at 52%.<\/p>\n<p>Drilling down as to why doctors plan to vote the way they do showed that most of those (91%) planning to vote \u201cno\u201d said the most influential factor was a belief that the economy will suffer under independence, followed by 72% who feared medical research will flounder.<!--more--><\/p>\n<p>\u201cYes\u201d voters believed that people will get better healthcare and social care under independence, and also wanted Scotland to have more autonomy about its healthcare system and how it is financed.<\/p>\n<p>Watching the experience of the English NHS has struck many doctors in Scotland. As one put it: \u201cSeeing how things are changing, and have changed, in the NHS in England, makes me thankful that health is devolved, and it seems that independence could make it easier for Scotland to tailor health and social care to what is needed, wanted, and works locally.\u201d<\/p>\n<p>Elsewhere on thebmj.com, consultant cardiac surgeon in Oxford <a href=\"http:\/\/www.bmj.com\/content\/349\/bmj.g5026\">Stephen Westaby tackles the complex issue of publishing deaths rates<\/a> of individual surgeons. He looks to the experience in the United States, where publication of cardiac surgeons\u2019 outcomes data in the 1980s led to gaming of the system and the shunning of high risk patients. Here in the UK, data on surgical outcomes should focus less on individual surgeons and reflect more comprehensively hospital procedures, the whole of the surgical team, and specific departments. \u201cPublication of SSMD [surgeon specific mortality data] has not improved patient choice, waiting lists, equipment, staffing, or team consistency,\u201d argues Wetsaby.<\/p>\n<p>According <a href=\"http:\/\/www.bmj.com\/content\/349\/bmj.g4934\">to an Editorial<\/a>, the question of whether to give tranexamic acid to patients undergoing joint replacement surgery remains unresolved, despite <a href=\"http:\/\/www.bmj.com\/content\/349\/bmj.g4829\">a study of 872 416 total hip or knee replacement procedures in the US<\/a>. The researchers acknowledge that their study has limitations, but they believe their findings warrant the identification and treatment of patients who are at high risk of bleeding and likely to need a blood transfusion. The editorialists are more cautious, however.
