What makes the difference? I\u2019m not sure we know, but I think it has something to do with resilience, obstreperousness, and grit.<\/p>\n<p>This week there has been a bit of a debate going on in our department over appropriate blood pressure targets for the elderly. This debate started in light of the eighth <a href=\"http:\/\/jama.jamanetwork.com\/article.aspx?articleid=1791497\">Joint National Committee (JNC 8) guidelines<\/a>, and has revolved mainly around balancing outcomes with adverse effects, NNTs, and patient engagement in medical decisions. As I\u2019ve observed (and participated just a bit in) this debate, in the back of my mind I keep thinking of my patients who do not fit the expectations of JNC 8\u2014or any other guidelines for that matter.<!--more--><\/p>\n<p>Reading JNC 8 or the <a href=\"http:\/\/circ.ahajournals.org\/content\/early\/2013\/11\/11\/01.cir.0000437738.63853.7a\">2013 American College of Cardiology\/ American Heart Association\u00a0guidelines<\/a>\u2014both of which I think are reasonably evidence based\u2014one gets the clear message that those found to be at risk, and who do not follow the recommended treatments, will have all sorts of horrible things happen to them: stroke, heart attack, and early death. Writ large, this ties into growing\u00a0concerns about the global \u201cepidemic\u201d of non-communicable chronic diseases.<\/p>\n<p>I\u2019ve certainly had my share of patients who have died untimely deaths due to chronic diseases, yet I\u2019ve also had those who seem to nearly completely ignore my advice . . . and yet they still keep on going and going and going. Despite uncontrolled blood pressure, high cholesterol, regular smoking, and lack of activity\u2014they keep on going!<\/p>\n<p>I\u2019ve joked more than once that such folks owe their longevity to their obstreperousness or cantankerousness, and often that seems to ring true. Not always, but at least sometimes it\u2019s the \u201ccharacters\u201d that just keep on going.<\/p>\n<p>\u201cResilience\u201d has been getting more and more press in the medical and psychological literature, and is defined as: &#8220;The process of negotiating, managing, and adapting to significant sources of stress or trauma . . . &#8216;bouncing back&#8217; in the face of adversity.&#8221; [<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3042897\/\">1<\/a>]<\/p>\n<p>This is postulated to be a more \u201casset based\u201d approach to understanding mental health, as opposed to \u201c&#8217;deficit&#8217; models of illness and psychopathology.\u201d So far so good, and I get the concept of \u201cbouncing back,\u201d but it turns out that we still do not know clearly how to operationalize, measure, or define it in practical terms [<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3042897\/\">1<\/a>]. What&#8217;s more, scales seeking to measure resilience all measure reactions, not proactivity.<\/p>\n<p>\u201cObstreperousness,\u201d by contrast, seems more \u201cpositive\u201d\u2014at least in the sense of being a description of one\u2019s initiated activity, not a description of reactivity, and it turns out we know even less about it. The only research I could find on \u201cobstreperousness\u201d is nearly 20 years old, and has to do with \u201caggressive, assaultive, motor\u201d behavior by demented patients [<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/1634698\">2<\/a>].<\/p>\n<p>\u201cGrit\u201d (the attitudinal kind, not the particulate kind) has been defined as \u201cperseverance and passion for long term goals\u201d [<a href=\"http:\/\/psycnet.apa.org\/journals\/psp\/92\/6\/1087\/\">3<\/a>], and has been found to be predictive of success among surgical residents [<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/24856121\">4<\/a>] and rural family doctors [<a href=\"http:\/\/www.jabfm.org\/content\/25\/6\/832.long\">5<\/a>]. So at least we know that docs with \u201cgrit\u201d seem to have \u201cstick-to-it-ive-ness.\u201d<\/p>\n<p>How do we relate this to our patients? The concepts of <a href=\"http:\/\/en.wikipedia.org\/wiki\/Motivational_interviewing\">motivational interviewing<\/a> are now being applied in <a href=\"http:\/\/archinte.jamanetwork.com\/article.aspx?articleid=1108789\">general medicine<\/a> in an effort to engage patients in understanding what makes themselves tick, and to encourage patients to move in a more healthy direction. Yet this is still focused on\u00a0engaging patients\u2019 intrinsic motivations, not with helping them find motivation, passion, or perseverance.<\/p>\n<p>Admittedly, at this point the discussion is becoming more about identity, spirituality, and direction in life\u2014but I think all these do have a bearing on who our patients are, where they go, and what they do with their lives.<\/p>\n<p>It\u2019s one thing to measure resilience, obstreperousness, or grit, and an entirely different thing to encourage or promote it. And none of this means I\u2019m going to forgo recommending evidence based treatments for chronic or acute conditions. I\u2019ll still recommend appropriate medications where the evidence indicates that they can improve morbidity and mortality, and I\u2019ll still encourage my patients to eat their fruits and veggies and get plenty of exercise.<\/p>\n<p>But I think, too, it\u2019s time for us to pay a bit more attention to those patients who are survivors\u2014those who keep going despite ignoring our well meant, evidence based advice. How do they do it? Passion? Perseverance?<\/p>\n<p>Perhaps those we find obstreperous or overly aggressive, may have the resilience and grit to keep on going.<\/p>\n<p>[1] Windle G, Bennett KM, Noyes J.
