To many people, including parts of the medical establishment, it seems counterintuitive to suggest that mammography might not be that effective and could lead to overdiagnosis. The evidence might be building, but it still doesn\u2019t cut it, they say. The latest piece of evidence was the BMJ paper by Anthony B Miller and colleagues, \u201c<a href=\"http:\/\/www.bmj.com\/content\/348\/bmj.g366\">Twenty five year follow-up for breast cancer incidence and mortality of the Canadian National Breast Screening Study: randomised screening trial<\/a>,&#8221; published exactly a month ago today. The authors concluded that annual mammography in women aged 40-59 does not reduce mortality from breast cancer beyond that of physical examination or usual care when adjuvant therapy for breast cancer is freely available. Four weeks on and 43 rapid responses later (not to mention the international press coverage and <a href=\"http:\/\/www.sciencebasedmedicine.org\/the-canadian-national-breast-screening-study-ignites-a-new-round-in-the-mammography-wars\/\">discussion in the blogosphere<\/a>), the controversy over the paper still rages. In the past 24 hours, <a href=\"http:\/\/www.bmj.com\/content\/348\/bmj.g366\/rr\/689698\">the <em>BMJ<\/em> has posted a lengthy response from the authors<\/a> to their critics. <a href=\"http:\/\/www.bmj.com\/content\/348\/bmj.g366\/rr\/689820\">One just-posted response<\/a> refers to the \u201cemotion and good intentions\u201d that the responder argues \u201ccan overcome a rational and courteous approach to the debate on breast screening.\u201d <!--more--><\/p>\n<p><a href=\"http:\/\/www.bmj.com\/content\/348\/bmj.g366\/rr\/689754\">And a responder from Uruguay<\/a>, where mammographic screening is compulsory for every female worker age 40-59, says: \u201cIf mammography screening had been a drug, it would have been withdrawn from the market.\u201d That responder is citing Peter C G\u00f8tzsche of the Nordic Cochrane centre, <a href=\"http:\/\/www.cmaj.ca\/content\/183\/17\/1957.full\">a longstanding critic of mammography screening programmes<\/a>, who has published much of his work on breast screening in the BMJ (<a href=\"http:\/\/www.bmj.com\/content\/338\/bmj.b86\">for example this research paper<\/a>).<\/p>\n<p>From mammography wars to real ones:<a href=\"http:\/\/www.bmj.com\/content\/348\/bmj.g1668\"> today we publish the obituary of Fred Vivian Griffiths<\/a>, a London general practitioner who flew Hurricanes and Spitfires in the second world war and who, remarkably, survived unscathed to undergo medical revalidation at the age of 90.
