At least in \u201cdeveloped\u201d or \u201chigher income\u201d countries, medical innovation seems inextricably tied with commercial endeavors, which often translate to more expensive ways to do the same things (sometimes, even if we\u2019re not sure the outcomes are truly better)<\/p>\n<p>The <a href=\"http:\/\/www.who.int\/bulletin\/volumes\/91\/1\/13-020113\/en\/index.html\">Bulletin of the World Health Organization<\/a> highlighted a different take on innovation in January, describing how \u201cfrugal innovation\u201d is helping drive both care and economic development in \u201cemerging economies.\u201d<\/p>\n<p>However, I think another area of &#8220;innovation&#8221; that needs attention, investigation, and promotion, is further developing the knowledge base as to when it is best to do nothing (which requires improving our knowledge of the natural course of diseases) or to take a less invasive approach. Some examples (admittedly maybe USA-centric, but that&#8217;s where I&#8217;m practicing&#8230;):<!--more--><\/p>\n<ul>\n<li>Mentholated ointment may be a good treatment for <a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/21209346\">onychomycosis<\/a>\u2014good to know, given the cost and potential side effects of standard antifungals.<\/li>\n<li><a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/12169503\">Bandaging alone<\/a> appears better than bandaging plus suturing for smaller hand lacerations.<\/li>\n<li><a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/9437338\">Vagal maneuvers are effective<\/a>\u00a0in terminating about one quarter of cases of spontaneous sumpraventricular tachycardia\u2014good to know if cardiac medications are not readily available.<\/li>\n<li>Postoperative ileus after colectomy may be <a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/22987303\">helped by coffee<\/a>\u00a0or <a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/17710495\">chewing gum<\/a>.<\/li>\n<li><a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/23319500\">Mean duration of cough for an acute respiratory illness is 17 days<\/a> (knowing the natural course can help frame expectations for resolution and when re-evaluation is or is not needed).<\/li>\n<\/ul>\n<p>Although it is important for all (medical professionals and the general public) to be informed as to what &#8220;medical&#8221; things work (ie, medications and chemicals, or interventions, surgeries, and procedures), it is just as important, and perhaps more so, for all to be informed when it is best to do nothing, or when something less expensive or invasive may be equivalent or better, or when it is just a matter of giving a benign condition time to run its course.<\/p>\n<p>How to develop this knowledge base, is an entirely different question!<\/p>\n<p>DECLARATION OF INTERESTS:<br \/>\n\u201cThis post is a revised version of thoughts posted to the <a href=\"http:\/\/www.hifa2015.org\/hifa2015-forum\/\">HIFA2015 email forum<\/a>.
