Adding science to the strident debate."},"content":{"rendered":"<h2>Guest blog by George Murley<\/h2>\n<p>There is an increasingly strident debate on the use of minimalist\/barefoot versus traditional sports footwear in running, and there <a href=\"http:\/\/www.therunningclinic.ca\/blog\/2012\/03\/discours-de-simon-bartold-analyse-simon-bartolds-speech-analysed\/\" target=\"_blank\">appear to be advocates for both sides who believe there is no need for a rational discussion<\/a>.<\/p>\n<figure id=\"attachment_2647\" aria-describedby=\"caption-attachment-2647\" style=\"width: 458px\" class=\"wp-caption aligncenter\"><img loading=\"lazy\" decoding=\"async\" class=\" wp-image-2647  \" src=\"https:\/\/blogs.bmj.com\/bjsm\/files\/2012\/03\/lieberman-screen-shot1.jpg\" alt=\"\" width=\"458\" height=\"248\" \/><figcaption id=\"caption-attachment-2647\" class=\"wp-caption-text\">Screen shot from: The Barefoot Professor - by Nature Video<\/figcaption><\/figure>\n<p><span style=\"color: #800080\"><strong>The debate<\/strong><\/span> appears to have escalated following publications by Richards and colleagues (2008) \u2018<em>Is your prescription of distance running shoes evidence-based?<\/em>\u2019 and later by Lieberman and colleagues (2009) \u2018<em>Foot strike patterns and collision forces in habitually barefoot versus shod runners<\/em>\u2019 and McDougall\u2019s book \u2014 \u2018<em>Born to Run.\u2019<\/em><\/p>\n<p><span style=\"color: #800080\"><strong>The main issue<\/strong><\/span> in this very messy debate seems to be whether &#8216;some&#8217; barefoot\/minimalist shoe running is beneficial. This is related to the first vertical impact force, minimalist shoes are meant encourages a forefoot strike and\u00a0 decrease this force, which in turn dampens the first vertical impact force. This however has some individuals suggesting that running barefoot may lead to injuries related to loading of the Achilles and direct impact of the forefoot. A second part of the argument is that footwear is supposed to weaken foot muscles whereas barefoot running challenges muscles and presumably leads to stronger\/hypertrophied muscles that in turn have a positive effect of function.<\/p>\n<p><span style=\"color: #800080\"><strong>Clinically<\/strong><\/span> we are primarily interested in the effect on injury.\u00a0 There are strong views and some limited evidence supporting arguments about the relationship between the first vertical impact force and injury.\u00a0 One perspective is that first vertical impact force causes injury whereas others argue injury is related to the &#8216;active&#8217; forces of push off.<\/p>\n<p><span style=\"color: #800080\"><strong>There are a ton of unanswered questions:<\/strong><\/span><\/p>\n<p>Does athletic shod or unshod running affect injury risk?<\/p>\n<p>How does shod and unshod running interact with comfort and performance?<\/p>\n<p>Which biomechanical parameters are related to injury risk?<\/p>\n<p>Does footwear or unshod running reverse biomechanics parameters related to injury risk?<\/p>\n<p><span style=\"color: #800080\"><strong>What is important<\/strong><\/span> is that clinicians and scientists approach this debate in a reasoned and calm way as there may be merit in both sides of the argument.
