The article by Tillett\u00a0and\u00a0Loosemore describes guidelines for the\u00a0prevention and management of travellers’ diarrhoea (TD) based on their\u00a0experience with the elite athletes and noncompeting members of Team\u00a0England during the 2008 Youth Commonwealth Games in India. The authors\u00a0recommended that all team members receive oral and written advice\u00a0regarding prevention of TD, that all team members are issued alcohol\u00a0hand\u00a0gel and instruction for its use, and that all noncompeting team members\u00a0receive ciprofloxacin for TD prophylaxis. As ciprofloxacin use in elite\u00a0athletes is considered controversial because of a possible association\u00a0with tendon rupture, the authors recommended that elite athletes\u00a0consider\u00a0the nonabsorbable antibiotic rifaximin as a prophylactic for TD.\u00a0However,\u00a0none of the elite athletes on Team England actually received rifaximin\u00a0as\u00a0a prophylactic therapy for TD. Further, the authors stopped short of\u00a0recommending rifaximin for the treatment of TD, simply recommending\u00a0treatment with empiric antibiotics per local advice and the results of\u00a0stool culture.<\/p>\n

We report here that, in 2008, some elite athletes from the\u00a0United\u00a0States received rifaximin either for the prophylaxis or treatment of TD\u00a0while in Beijing, China. In this small sample of elite athletes,\u00a0rifaximin\u00a0was safe and well tolerated, and no adverse events were reported.\u00a0Rifaximin has been found safe, well tolerated, and effective for both\u00a0the\u00a0prophylaxis and treatment of TD in other populations1-8. Based on our\u00a0experience and the excellent safety profile of rifaximin for the\u00a0treatment\u00a0of TD, the use of rifaximin as an antibiotic therapy for the treatment\u00a0of\u00a0TD in elite athletes deserves further consideration.<\/p>\n
