{"id":11896,"date":"2026-06-17T13:48:02","date_gmt":"2026-06-17T12:48:02","guid":{"rendered":"https:\/\/blogs.bmj.com\/bjsm\/?p=11896"},"modified":"2026-06-17T13:48:02","modified_gmt":"2026-06-17T12:48:02","slug":"the-pain-isnt-the-hardest-challenge-agency-not-anatomy-drives-recovery-from-persisting-low-back-pain-in-elite-athletes","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/bjsm\/2026\/06\/17\/the-pain-isnt-the-hardest-challenge-agency-not-anatomy-drives-recovery-from-persisting-low-back-pain-in-elite-athletes\/","title":{"rendered":"\u201cThe pain isn\u2019t the hardest challenge\u2026\u201d Agency, not anatomy drives recovery from persisting low back pain in elite athletes"},"content":{"rendered":"<p><em><strong>Authors<\/strong>: Trease, L for all authors &#8211; Foley, G; Kemp, JL; Hancock, MJ; Wilson, F; Makdissi, M; Morrison, J; Mosler, A<\/em><\/p>\n<h3>Why is this study important?<\/h3>\n<p><span style=\"font-weight: 400\">Low back pain (LBP) is common in elite sport, affecting around half of all athletes each year (<\/span><span style=\"font-weight: 400\">1).<\/span><span style=\"font-weight: 400\"> Currently, most research focuses on risk factors, anatomy and biomechanics (<\/span><span style=\"font-weight: 400\">2, 3)<\/span><span style=\"font-weight: 400\">, with treatment studies largely centred on biological interventions (4,<\/span>\u00a0<span style=\"font-weight: 400\">5).<\/span><span style=\"font-weight: 400\"> This approach is disconnected from the broader community LBP literature where a biopsychosocial approach is considered best practice (6).\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Our study explored how elite athletes experience pLBP and the biopsychosocial factors that shape their recovery.<\/span><\/p>\n<h3>How did the study go about this?<\/h3>\n<p><span style=\"font-weight: 400\">We conducted in-depth, semi-structured interviews with 17 Australian elite athletes competing across 10 Olympic sports. All had LBP lasting longer than three months. Using reflexive thematic analysis, we explored their lived experiences: what pain meant to them, what shaped their recovery, and how their sporting environment influenced both. Importantly, athletes were involved throughout the research process, ensuring the findings remained grounded in real-world elite sport.<\/span><\/p>\n<h3>What did the study find?<\/h3>\n<p><span style=\"font-weight: 400\">Four themes captured how athletes experienced and navigated pLBP.<\/span><\/p>\n<ol>\n<li><b> The burden of pLBP extends beyond pain<\/b><\/li>\n<\/ol>\n<p><span style=\"font-weight: 400\">Athletes described pLBP as intrusive, with impacts extending well beyond physical symptoms. Unlike training pain, which is expected and chosen, pLBP was unpredictable and disruptive. It eroded confidence, challenged identity and created a sense of losing control over the body, \u201can alien presence\u201d. For many, the hardest part was not the pain itself, but what it meant for their ability to train, compete and maintain a sense of self.<\/span><\/p>\n<ol start=\"2\">\n<li><b> Validation drives agency\u2014and agency drives recovery<\/b><\/li>\n<\/ol>\n<p><span style=\"font-weight: 400\">Validation was central to recovery. When clinicians validated an athlete\u2019s pain, and helped them understand it, athletes felt more in control. In contrast, uncertainty or dismissal eroded confidence and trust. Diagnostic labelling and clear explanations often acted as a turning point, reducing fear and increasing engagement in rehab. This validation appeared to underpin agency.\u00a0<\/span><\/p>\n<ol start=\"3\">\n<li><b> Mental health and pain are tightly intertwined<\/b><\/li>\n<\/ol>\n<p><span style=\"font-weight: 400\">Athletes described a bidirectional relationship between pain and psychological distress. Periods of high pain often coincided with low mood, anxiety, and fear of re-injury. But those who could reframe pain as manageable\u2014not threatening\u2014were better able to stay engaged in training and recovery.<\/span><\/p>\n<ol start=\"4\">\n<li><b> Culture and relationships shape recovery<\/b><\/li>\n<\/ol>\n<p><span style=\"font-weight: 400\">Pain was experienced within, and shaped by, the elite sporting environment. Athletes described pressure to continue training, fear of being perceived as weak or replaceable, and reluctance to disclose symptoms. Relationships with coaches, clinicians and teammates strongly influenced these experiences. Support outside sport, particularly from family and partners, was often positive for pain experience.<\/span><\/p>\n<h3>What are the key take-home points?<\/h3>\n<p><span style=\"font-weight: 400\">Elite sport is a unique environment where self-worth, selection, and income are closely tied to health and performance. When pain persists, it doesn\u2019t just affect training, it can threaten athlete identity, belonging and career trajectory.<\/span><\/p>\n<p><span style=\"font-weight: 400\">For clinicians, this means it\u2019s not enough to prescribe exercises or interpret scans. We must:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Communicate clearly and contextually\u00a0<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Validate the athlete\u2019s pain experience\u00a0<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Facilitate shared decision-making\u00a0<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Recognise the pressures of elite sport\u00a0<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Advocate for environments where athletes feel safe to disclose pain\u00a0<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400\">In elite sport, recovery is not just about getting athletes back to performance\u2014it\u2019s about protecting their autonomy, identity, and well-being along the way.
