Sports medicine physicians (SMPs) are often the first to notice\u00a0when something is wrong, so they play a key role in protecting athletes. But until\u00a0now, we didn\u2019t know how confident SMPs feel about recognizing and reporting\u00a0harassment and abuse. This study is the first to look at how prepared SMPs\u00a0around the world are to handle these situations and highlights important gaps that\u00a0need addressing (1).<\/p>\n<p><strong>How did the study go about this?<\/strong><\/p>\n<p>We surveyed 406 sports medicine physicians from 115 countries to find out how well\u00a0they can identify and report harassment and abuse in their athletes. We also asked\u00a0about their experiences over the past year, how confident they feel about dealing\u00a0with these issues, and what barriers get in the way of reporting.<\/p>\n<p><strong>What did the study find?<\/strong><\/p>\n<p>Even though most SMPs are aware of the harms caused by harassment and abuse,\u00a0many don\u2019t feel comfortable recognizing the signs or reporting their concerns. Over\u00a0half of the physicians said they hadn\u2019t received enough training, and nearly 85%\u00a0want more education in this area. A lot of them didn\u2019t know where or how to report\u00a0harassment and abuse, and some didn\u2019t know who the safeguarding officer in their\u00a0organization was.\u00a0Interestingly, female SMPs showed a greater awareness of the impacts of\u00a0harassment, which may be linked to their own experiences in sport. However, both\u00a0male and female physicians pointed out similar barriers to reporting, such as worries\u00a0about confidentiality, fear of backlash, and lack of time or knowledge.\u00a0Despite increasing awareness of harassment and abuse in sports, this study reveals\u00a0a critical gap in the training and confidence of SMPs worldwide. While physicians\u00a0recognize the need to protect athletes, practical barriers\u2014like fears of breaching\u00a0confidentiality, potential retaliation, and a lack of clarity on reporting\u00a0procedures\u2014pose challenges. These obstacles highlight a disconnect between\u00a0understanding the severity of HA and feeling equipped to act on suspicions.\u00a0Addressing these barriers is essential for creating a safer environment for athletes.\u00a0Moreover, the gender differences observed emphasize the need for more nuanced\u00a0training. Female SMPs, likely drawing from personal or professional experiences,\u00a0demonstrated greater sensitivity to the harms of harassment and abuse. This points\u00a0to the importance of including diverse perspectives and lived experiences in\u00a0safeguarding education.<\/p>\n<p><strong>What are the key take-home points?<\/strong><\/p>\n<ol>\n<li>Need for more training: Over half of the SMPs feel they don\u2019t have enough<br \/>\ntraining to deal with harassment and abuse, and nearly all want more<br \/>\neducation to help them develop these skills.<\/li>\n<li>Barriers to reporting: Common reasons for not reporting include worries<br \/>\nabout confidentiality, fear of negative consequences, and not knowing where<br \/>\nto report or who to talk to.<\/li>\n<li>Gender differences: Female SMPs were more aware of the impact of<br \/>\nharassment and abuse, possibly because of their own experiences. This<br \/>\nshows the need for gender-sensitive training.<\/li>\n<li>Call for action: Safeguarding training should be a standard part of both initial<br \/>\nand ongoing training for SMPs. There\u2019s also a need for an international\u00a0safeguarding code to guide SMPs on how to protect athletes and report\u00a0harassment and abuse.<\/li>\n<\/ol>\n<p><strong>How should this change clinical practice?<\/strong><\/p>\n<p>SMPs need to feel confident in recognizing and responding to harassment and\u00a0abuse. To achieve this, safeguarding training needs to be included in sports\u00a0medicine programs and in continuing medical education. This study also shows that\u00a0training needs to address gender and cultural differences.
