We also provide our recommendations for planning future studies. The <\/span><a href=\"https:\/\/bjsm.bmj.com\/content\/early\/2023\/02\/22\/bjsports-2022-106340\"><span style=\"font-weight: 400\">study<\/span><\/a><span style=\"font-weight: 400\"> was recently published in the BJSM.\u00a0\u00a0<\/span><\/p>\n<p><b>Why is this study important?<\/b><\/p>\n<p><span style=\"font-weight: 400\">If you are treating shoulder patients, you probably see many diagnosed with SAPS, shoulder impingement, and rotator cuff syndrome \u2013 or are they the same? It is important to have a common understanding of both terminology and diagnostic criteria to avoid confusion, communicate efficiently and navigate the literature. <\/span><\/p>\n<p><span style=\"font-weight: 400\"><br \/>\n<\/span><span style=\"font-weight: 400\">SAPS is regarded as the most common cause of shoulder pain. Despite this, there is no universally recognised terminology or diagnostic criteria. An overview of trends in terminology can help us understand the ongoing discussion on the use of diagnostic labels, and aid communication among scientists and healthcare professionals. Differences in diagnostic criteria can foster misconceptions &#8211; if we do not agree on the definition of a problem, we cannot agree on the solution. To enable a qualified interpretation of the literature, it is important to have an overview and understanding of the terminology and diagnostic criteria used in the literature. You need to know the past to understand the present.<\/span><span style=\"font-weight: 400\"><br \/>\n<\/span><\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"alignnone wp-image-10545\" src=\"https:\/\/blogs.bmj.com\/bjsm\/files\/2023\/04\/shoulder-1-300x156.png\" alt=\"\" width=\"528\" height=\"275\" srcset=\"https:\/\/blogs.bmj.com\/bjsm\/files\/2023\/04\/shoulder-1-300x156.png 300w, https:\/\/blogs.bmj.com\/bjsm\/files\/2023\/04\/shoulder-1.png 512w\" sizes=\"auto, (max-width: 528px) 100vw, 528px\" \/><\/p>\n<p><span style=\"font-weight: 400\">Figure 1: Terminology used in the period 1972\u20132019 to describe patients with subacromial pain syndrome.<\/span><\/p>\n<p><b>How did the study go about this?<\/b><\/p>\n<p><span style=\"font-weight: 400\">We did a comprehensive review, and mapped the terminology and the diagnostic criteria used in studies investigating SAPS.\u00a0 \u00a0 <\/span><span style=\"font-weight: 400\"><br \/>\n<\/span><span style=\"font-weight: 400\"><br \/>\n<\/span><b>What did the study find?<\/b><\/p>\n<p><b><\/b><span style=\"font-weight: 400\">\u2219<\/span> <span style=\"font-weight: 400\">The terminology used to describe patients with SAPS varies considerably across studies and time. More mechanistic terms containing \u2018impingement\u2019 are used less than before, while SAPS is used increasingly. <\/span><span style=\"font-weight: 400\"><br \/>\n<\/span><span style=\"font-weight: 400\"><br \/>\n<\/span><span style=\"font-weight: 400\">\u2219<\/span> <span style=\"font-weight: 400\">The diagnostic criteria were often based on physical examination tests, but many studies did not report any diagnostic criteria. Combinations of Hawkin\u2019s, Neer\u2019s, Jobe\u2019s, painful arc, injection test and isometric shoulder strength tests were most commonly used, though this varied considerably. <\/span><span style=\"font-weight: 400\"><br \/>\n<\/span><span style=\"font-weight: 400\"><br \/>\n<\/span><span style=\"font-weight: 400\">\u2219<\/span> <span style=\"font-weight: 400\">Imaging was most often used to exclude other pathology, but it was not used consistently. <\/span><span style=\"font-weight: 400\"><br \/>\n<\/span><\/p>\n<p><span style=\"font-weight: 400\">\u2219<\/span> <span style=\"font-weight: 400\">Patients with full-thickness supraspinatus tears were often excluded, however, many studies did not report whether they included or excluded these patients.\u00a0<\/span><\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"alignnone wp-image-10547\" src=\"https:\/\/blogs.bmj.com\/bjsm\/files\/2023\/04\/shoulder-2-1-198x300.png\" alt=\"\" width=\"337\" height=\"511\" srcset=\"https:\/\/blogs.bmj.com\/bjsm\/files\/2023\/04\/shoulder-2-1-198x300.png 198w, https:\/\/blogs.bmj.com\/bjsm\/files\/2023\/04\/shoulder-2-1.png 313w\" sizes=\"auto, (max-width: 337px) 100vw, 337px\" \/><\/p>\n<p>Table 1: Physical examination tests and imaging modalities used to diagnose subacromial pain syndrome.<\/p>\n<p><b>What are the key take-home points?<\/b><\/p>\n<p><b><\/b><span style=\"font-weight: 400\">\u2219<\/span> <span style=\"font-weight: 400\">Studies investigating SAPS are heterogeneous to an extent that makes it difficult, and often impossible, to compare studies. <\/span><span style=\"font-weight: 400\"><br \/>\n<\/span><span style=\"font-weight: 400\"><br \/>\n<\/span><span style=\"font-weight: 400\">\u2219<\/span> <span style=\"font-weight: 400\">Future studies should have a clear definition of the patient population, and apply a systematic methodological approach, to ensure reproducibility and aid in the interpretation and generalisability of the findings.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">\u2219<\/span> <span style=\"font-weight: 400\">We recommend using the term SAPS. The word \u2018subacromial pain\u2019 encapsulates the cardinal symptom of SAPS, and the word \u2018syndrome\u2019 recognises that the pathophysiology is not fully understood.<\/span><span style=\"font-weight: 400\"><br \/>\n<\/span><span style=\"font-weight: 400\"><br \/>\n<\/span><span style=\"font-weight: 400\">\u2219<\/span> <span style=\"font-weight: 400\">When classifying patients with SAPS, we recommend using a combination of physical\u00a0 examinations tests, and imaging, as other potential shoulder pathology, that can mimic the symptoms of SAPS, must be excluded.
