Established in 2002 by Agita Mundo with support from the World Health Organization (WHO), it is a perfect opportunity to reflect on where we are around the world and highlight bright spots for improvement. Here we present a Canadian example of answering the WHO\u2019s call to action on physical activity implementation.\u00a0<\/span><\/p>\n<h6><strong>Infographic 1<\/strong>: Every move counts<\/h6>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"alignnone wp-image-10519\" src=\"https:\/\/blogs.bmj.com\/bjsm\/files\/2023\/04\/Every-move-counts.-jpeg-300x125.jpg\" alt=\"\" width=\"676\" height=\"282\" srcset=\"https:\/\/blogs.bmj.com\/bjsm\/files\/2023\/04\/Every-move-counts.-jpeg-300x125.jpg 300w, https:\/\/blogs.bmj.com\/bjsm\/files\/2023\/04\/Every-move-counts.-jpeg-768x319.jpg 768w, https:\/\/blogs.bmj.com\/bjsm\/files\/2023\/04\/Every-move-counts.-jpeg-640x266.jpg 640w, https:\/\/blogs.bmj.com\/bjsm\/files\/2023\/04\/Every-move-counts.-jpeg.jpg 1526w\" sizes=\"auto, (max-width: 676px) 100vw, 676px\" \/><\/p>\n<p>&nbsp;<\/p>\n<h4><b>The Problem \u2013 Our Collective Physical Inactivity\u00a0<\/b><\/h4>\n<p><span style=\"font-weight: 400\">Physical activity benefits health in many ways at any age, playing a vital role in treatment and prevention of noncommunicable diseases, but we aren\u2019t moving nearly enough:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Half (49%) of Canadian adults (18\u201379 years) and fewer than half (44%) of children and youth (5\u201317 years) meet national physical activity guidelines for their health (1).<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Worldwide, it is estimated that physical inactivity causes 6% of the burden of disease from coronary heart disease, 7% of type 2 diabetes, 10% of breast cancers and 10% of colon cancers (2). Physical inactivity is also associated with a higher risk of severe COVID-19 outcomes (3,4) and a higher likelihood of infection (5).<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Our collective inactivity is one of the largest risk factors for early morbidity and mortality. A 25% reduction in the rate of physical inactivity would prevent an estimated 1.3 million deaths a year globally (2).\u00a0<\/span><\/li>\n<\/ul>\n<h6><strong>Infographic 2:<\/strong> Physical Activity Prevalence in 2020<\/h6>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"alignnone wp-image-10520\" src=\"https:\/\/blogs.bmj.com\/bjsm\/files\/2023\/04\/Meeting-Guidelines--300x186.jpeg\" alt=\"\" width=\"639\" height=\"396\" srcset=\"https:\/\/blogs.bmj.com\/bjsm\/files\/2023\/04\/Meeting-Guidelines--300x186.jpeg 300w, https:\/\/blogs.bmj.com\/bjsm\/files\/2023\/04\/Meeting-Guidelines--768x477.jpeg 768w, https:\/\/blogs.bmj.com\/bjsm\/files\/2023\/04\/Meeting-Guidelines--640x398.jpeg 640w, https:\/\/blogs.bmj.com\/bjsm\/files\/2023\/04\/Meeting-Guidelines-.jpeg 1274w\" sizes=\"auto, (max-width: 639px) 100vw, 639px\" \/><\/p>\n<h4><b>World Health Organization\u2019s Global Action Plan on Physical Activity<\/b><\/h4>\n<p><span style=\"font-weight: 400\">The World Health Organization (WHO) launched the <\/span><a href=\"https:\/\/www.who.int\/news-room\/initiatives\/gappa\/action-plan\"><span style=\"font-weight: 400\">Global Action Plan on Physical Activity (GAPPA)<\/span><\/a><span style=\"font-weight: 400\"> in 2018 to deal with this global epidemic, helping countries prioritize decreasing physical inactivity. The WHO\u2019s target is to reduce the global prevalence of physical inactivity in adults and adolescents by 15% by 2030. The WHO released updated <\/span><a href=\"https:\/\/bjsm.bmj.com\/content\/54\/24\/1451\"><span style=\"font-weight: 400\">global physical activity guidelines<\/span><\/a><span style=\"font-weight: 400\"> to help countries implement the GAPPA.<\/span><\/p>\n<blockquote>\n<p style=\"text-align: center\"><em><strong>Implementation<\/strong> includes strategies to promote the adoption and integration of evidence-based health interventions into practice. Implementation research includes, but is not limited to, examining factors affecting implementation, implementation processes, methodologies and outcomes. \u00a0The intent is to understand what, why and how interventions work in &#8220;real world&#8221; settings.<\/em><\/p>\n<\/blockquote>\n<h4><b>The Project &#8211; Western Research Hub for Physical Activity and Health<\/b><\/h4>\n<p><span style=\"font-weight: 400\">The Western Research Hub for Physical Activity and Health is our response to this call to action. Our mission is: <\/span><b>to advocate for and implement meaningful, sustainable and culturally relevant physical activity opportunities within our community and beyond.<\/b><\/p>\n<p><span style=\"font-weight: 400\">Our established team of over 30 diverse junior and senior scholars from seven different Faculties at Western University in London, Ontario, Canada, and a Community Advisory Council of members made up of six local organization representatives and four patient partners who co-design and inform our work. As Director of the Hub, I work closely with our Community Advisory Council (CAC) and research team, developing and implementing physical activity research accountable to our mission.<\/span><\/p>\n<h5><b>\u201cNothing about us, without us\u201d: the importance of a Community Advisory Council\u00a0<\/b><\/h5>\n<p><span style=\"font-weight: 400\">Our CAC addresses the GAPPA\u2019s call for a \u201cwhole of system\u201d response, including multisectoral partnerships and meaningful community engagement to tackle the physical inactivity epidemic. We selected CAC members through an interview process, seeking to identify and recruit diverse perspectives from individuals with lived experience, members of under-represented groups, and community organisation representatives. The CAC meets regularly with our research team, bringing important and varied perspectives to co-design our implementation strategy using an integrated approach.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Reflections from CAC members indicate key motivating factors for participation:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">being part of research team, shaping research questions, contributing to the decision-making process<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">giving feedback from their unique perspectives<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">flexible meeting times and a direct line of communication with researchers making participation easier<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">feeling valued through honoraria<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400\">These factors are summarized in our <\/span><a href=\"https:\/\/doi.org\/10.1136\/bjsports-2022-106190\"><span style=\"font-weight: 400\">Patient Engagement Model<\/span><\/a><span style=\"font-weight: 400\"> (Figure 1), a direct result of our work with the CAC.<\/span><\/p>\n<h6><strong>Figure 1<\/strong>: Patient engagement model<\/h6>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"alignnone wp-image-10521\" src=\"https:\/\/blogs.bmj.com\/bjsm\/files\/2023\/04\/Screenshot-2023-04-05-at-17.40.31-300x184.png\" alt=\"\" width=\"693\" height=\"425\" srcset=\"https:\/\/blogs.bmj.com\/bjsm\/files\/2023\/04\/Screenshot-2023-04-05-at-17.40.31-300x184.png 300w, https:\/\/blogs.bmj.com\/bjsm\/files\/2023\/04\/Screenshot-2023-04-05-at-17.40.31-768x472.png 768w, https:\/\/blogs.bmj.com\/bjsm\/files\/2023\/04\/Screenshot-2023-04-05-at-17.40.31-1536x944.png 1536w, https:\/\/blogs.bmj.com\/bjsm\/files\/2023\/04\/Screenshot-2023-04-05-at-17.40.31-640x393.png 640w, https:\/\/blogs.bmj.com\/bjsm\/files\/2023\/04\/Screenshot-2023-04-05-at-17.40.31.png 1566w\" sizes=\"auto, (max-width: 693px) 100vw, 693px\" \/><\/p>\n<p>&nbsp;<\/p>\n<h4><b>Building Capacity for Implementation<\/b><\/h4>\n<p><span style=\"font-weight: 400\">Over the next 2 years, the Hub will address 8 priorities as identified and voted on by our community (Figure 2):<\/span><\/p>\n<p><strong>Figure 2<\/strong>:\u00a0<span style=\"font-weight: 400\">Community-led priorities for physical activity implementation<\/span><\/p>\n<blockquote><p><br style=\"font-weight: 400\" \/><em><img loading=\"lazy\" decoding=\"async\" class=\"alignnone wp-image-10516 aligncenter\" src=\"https:\/\/blogs.bmj.com\/bjsm\/files\/2023\/04\/PA-day-2-300x57.png\" alt=\"\" width=\"647\" height=\"123\" srcset=\"https:\/\/blogs.bmj.com\/bjsm\/files\/2023\/04\/PA-day-2-300x57.png 300w, https:\/\/blogs.bmj.com\/bjsm\/files\/2023\/04\/PA-day-2.png 512w\" sizes=\"auto, (max-width: 647px) 100vw, 647px\" \/><\/em><\/p>\n<p style=\"text-align: center\"><em><span style=\"font-weight: 400\">(1) engage and establish partnerships with key stakeholders<\/span><\/em><\/p>\n<p style=\"text-align: center\"><em><span style=\"font-weight: 400\">(2) harmonize messaging about physical activity among healthcare providers\u00a0<\/span><\/em><\/p>\n<p style=\"text-align: center\"><em><span style=\"font-weight: 400\">(3) explore post-pandemic opportunities to support mental health of children\u00a0<\/span><\/em><\/p>\n<p style=\"text-align: center\"><em><span style=\"font-weight: 400\">(4) address infrastructural barriers to physical activity<\/span><\/em><\/p>\n<p style=\"text-align: center\"><em><span style=\"font-weight: 400\">(5) invest and promote active travel to schools and workplaces<\/span><\/em><\/p>\n<p style=\"text-align: center\"><em><span style=\"font-weight: 400\">(6) explore the use of technologies &amp; digital tools to promote physical activity equitably<\/span><\/em><\/p>\n<p style=\"text-align: center\"><em><span style=\"font-weight: 400\">(7) support social connectedness and mental health of older adults through physical activity programming in community and institutional settings<\/span><\/em><\/p>\n<p style=\"text-align: center\"><em><span style=\"font-weight: 400\">(8) ensure that individuals receiving institutional care can access equipment and resources to support movement and physical activity<\/span><\/em><\/p>\n<\/blockquote>\n<p><span style=\"font-weight: 400\">Our <\/span><b>Implementation Support Service<\/b><span style=\"font-weight: 400\"> pilot will provide access to tools and resources for interested researchers and community members to build capacity, strengthen partnerships and increase potential for successful implementation projects.
