{"id":990,"date":"2014-10-16T20:53:25","date_gmt":"2014-10-16T19:53:25","guid":{"rendered":"https:\/\/blogs.bmj.com\/adc\/?p=990"},"modified":"2015-11-04T15:06:06","modified_gmt":"2015-11-04T14:06:06","slug":"why-not-look-at-what-you-already-know","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/adc\/2014\/10\/16\/why-not-look-at-what-you-already-know\/","title":{"rendered":"Why not look at what you already know?"},"content":{"rendered":"<p><img loading=\"lazy\" decoding=\"async\" class=\"alignleft\" src=\"http:\/\/www.birmingham.ac.uk\/Images\/College-MDS-only\/HaPS\/research\/projects\/MHTMR\/Logo.jpg\" alt=\"\" width=\"172\" height=\"60\" \/>A little while ago we blogged on the surprisingly varied methods folk use to pick how <a href=\"https:\/\/blogs.bmj.com\/adc\/2014\/05\/14\/underpowered-and-over-here\/\">how big an effect needs to be in order to be &#8216;clinically relevant&#8217;.<\/a> A further paper on this theme has emerged that takes up a slightly different aspect of the challenge of getting the number right before doing a trial.<\/p>\n<p>On the basics front, before you know how many people will be needed for a trial, you need to know<\/p>\n<ul>\n<li>How big an effect you might see<\/li>\n<li>How varied the\u00a0effect is between people<\/li>\n<li>What size of effect is gong to be &#8216;clinically relevant&#8217; (ie above what level you want to prove the intervention will lie)<\/li>\n<li><a href=\"https:\/\/blogs.bmj.com\/adc\/2013\/09\/12\/statsminiblog-type-i-and-ii-errors\/\">What chance of making the wrong call<\/a> (&#8220;It works!&#8221; when it doesn&#8217;t, or vica versa) you are prepared to accept<\/li>\n<\/ul>\n<p>It may be rather surprising to find that\u00a0there hasn&#8217;t been, until very recently, a really well developed way of using systematic review \/ meta-analysis methodology to capture the stuff we already know before moving onwards to find out more, when moving between phase II (how-toxic-is-this-and-does-it-make-markers\/images-better?) and phase III (are-there-fewer-dead-people?) trials. But now there is.<\/p>\n<p><!--more--><\/p>\n<p>A <a href=\"http:\/\/www.trialsjournal.com\/content\/15\/1\/346\">group in Birmingham (UK) have looked in great detail at the problem<\/a>. They use an example from thrombolytic therapy, they have captured six key things to think about while you&#8217;re doing your systematic review and power-preparing meta-analysis:<\/p>\n<table id=\"poparticle\" border=\"0\" width=\"100%\" rules=\"none\" cellspacing=\"5\" cellpadding=\"0\">\n<tbody align=\"left\">\n<tr>\n<td class=\"rowsep0\">\n<pre>Framework<\/pre>\n<\/td>\n<td class=\"rowsep0\">\n<pre>Use a <a title=\"StatsMiniBlog: Rethinking meta-analysis\" href=\"https:\/\/blogs.bmj.com\/adc\/2014\/06\/15\/statsminiblog-rethinking-meta-analysis\/\">binomial logistic regression model<\/a> : avoid the \"zero event\" problems for small studies<\/pre>\n<\/td>\n<\/tr>\n<tr>\n<td class=\"rowsep0\">\n<pre>Choice of model<\/pre>\n<\/td>\n<td class=\"rowsep0\">\n<pre>Make decision on using random (almost always) or fixed effects meta-analysis based on good clinical thinking ... \"same drug \/ dose \/ outcome \/ duration? ... if 'No' then it's random\"<\/pre>\n<\/td>\n<\/tr>\n<tr>\n<td class=\"rowsep0\">\n<pre>Heterogeneity<\/pre>\n<\/td>\n<td class=\"rowsep0\">\n<pre>Following on from this, <a title=\"It\u2019s how mixed up? Meta analysis models step one.\" href=\"https:\/\/blogs.bmj.com\/adc\/2011\/03\/27\/its-how-mixed-up-meta-analysis-models-step-one\/\">expect and incorporate\u00a0heterogeneity<\/a> in treatment effects by estimating wide<\/pre>\n<\/td>\n<\/tr>\n<tr>\n<td class=\"rowsep0\">\n<pre>Uncertainty<\/pre>\n<\/td>\n<td class=\"rowsep0\">\n<pre>Use a Bayesian framework to model\u00a0parameter uncertainty (<a title=\"It\u2019s how ineffective?\" href=\"https:\/\/blogs.bmj.com\/adc\/2010\/06\/06\/its-how-ineffective\/\">confidence intervals around effect size<\/a>) and external evidence (such as the between-study differences) BUT\u00a0sensitivity analysis to the choice of prior distributions is required. (OK - so this step is a bit too complex for a mini-blog to cover just yet...)<\/pre>\n<\/td>\n<\/tr>\n<tr>\n<td class=\"rowsep0\">\n<pre>Prediction intervals<\/pre>\n<\/td>\n<td class=\"rowsep0\">\n<pre>Report <a title=\"Confident in predicting? Meta analysis models step two.\" href=\"https:\/\/blogs.bmj.com\/adc\/2011\/03\/27\/confident-in-predicting-meta-analysis-models-step-two\/\">95% prediction intervals<\/a> 'cause you want to know the potential answers from new trials in different populations - your\u00a0subsequent Phase III study.<\/pre>\n<\/td>\n<\/tr>\n<tr>\n<td class=\"rowsep1\">\n<pre>Bias<\/pre>\n<\/td>\n<td class=\"rowsep1\">\n<pre>Consider using\u00a0skeptical prior distributions for treatment if you think the\u00a0Phase II trials may be biased.<\/pre>\n<\/td>\n<\/tr>\n<\/tbody>\n<\/table>\n<p>This all makes tonnes of sense, even if it is written in statistics:<\/p>\n<ol>\n<li>The phase II studies are going\u00a0to be done in very diverse populations, and the &#8216;average&#8217; answer might well belie a truth that sits differently for different groups &#8211; so use a <a title=\"Confident in predicting? Meta analysis models step two.\" href=\"https:\/\/blogs.bmj.com\/adc\/2011\/03\/27\/confident-in-predicting-meta-analysis-models-step-two\/\">random effects model and report prediction intervals<\/a><\/li>\n<li>Small phase II studies might well be optimised to collect and report if things go well (we know that\u00a0<a href=\"http:\/\/laikaspoetnik.wordpress.com\/2011\/09\/08\/futon-bias-or-why-limiting-to-free-full-text-might-not-always-be-a-good-idea\/\">publication bias is a massive problem<\/a> and that <a href=\"https:\/\/blogs.bmj.com\/adc\/2013\/03\/10\/missing-presumed-suppressed\/\">not all things are shared openly<\/a>) so account for this<\/li>\n<li>Small studies might end up with 0 events. And we know that<a title=\"Harms, safety and certainties\" href=\"https:\/\/blogs.bmj.com\/adc\/2013\/03\/06\/harms-safety-and-certainties\/\"> 0 is NOT &#8216;doesn&#8217;t happen&#8217;<\/a>. So use the right maths to model it with binomial approaches.<\/li>\n<\/ol>\n<p>What I&#8217;d love to see now is a bunch of folk looking to see if\u00a0we had done phase II meta-analysis like this would we\u00a0have undertaken some phase III trials with more honest appreciation of how likely they are to be correct?
