The <a href=\"https:\/\/twitter.com\/hashtag\/realebm\">#RealEBM<\/a> hashtag (go on &#8230; give it a go ..) is addressing this quite eloquently and has been graven in stone by the superb <a class=\"ProfileHeaderCard-screennameLink u-linkComplex js-nav\" href=\"https:\/\/twitter.com\/RichardLehman1\">@<span class=\"u-linkComplex-target\">RichardLehman1<\/span><\/a> in <a href=\"http:\/\/bjgp.org\/content\/65\/639\/532\">Ten Commandments<\/a>*<\/p>\n<p>There&#8217;s another triple target too, that I bumped into quite recently while doing more <a href=\"http:\/\/pediatrics.aappublications.org\/content\/134\/6\/e1648\">work on transition from child into adult health<\/a> services. This is the aim for transition services to<\/p>\n<ul>\n<li>improve\u00a0patient experience<\/li>\n<li>increase community health<\/li>\n<li>have an appropriate and affordable cost<\/li>\n<\/ul>\n<p>Why on earth have I not considered before that these three aims are\u00a0core to EVERY health care system that we undertake?<\/p>\n<p><!--more--><\/p>\n<p>And challengingly, <a href=\"https:\/\/blogs.bmj.com\/adc\/2015\/10\/02\/clashing-concepts\/\">what do we do when they clash<\/a>?<\/p>\n<p>There&#8217;s certainly something\u00a0rather\u00a0stale about arguments on cost-effectiveness of new drugs or technologies, the viewpoints of society vs. individuals, and the currently accepted view that we do need to balance out costs against benefits to make health care systems work effectively.<\/p>\n<p>I&#8217;m not sure there&#8217;s a debate at the moment about how we balance issues of patient experience (another good hashtag <a href=\"https:\/\/twitter.com\/hashtag\/cypexp\">#CYPexp<\/a>) against cost, or against community health gains yet. Perhaps this is right &#8211; while we still <a href=\"http:\/\/adc.bmj.com\/content\/early\/2015\/06\/04\/archdischild-2014-307492.extract\">roundly ignore integrating the\u00a0voices of young people<\/a> into research, practice and policy we should not be debating limits and competing pressures.
