{"id":1106,"date":"2015-06-16T20:50:50","date_gmt":"2015-06-16T19:50:50","guid":{"rendered":"https:\/\/blogs.bmj.com\/adc\/?p=1106"},"modified":"2015-05-31T20:08:20","modified_gmt":"2015-05-31T19:08:20","slug":"practical-authorship-should-i-write-a-case-report","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/adc\/2015\/06\/16\/practical-authorship-should-i-write-a-case-report\/","title":{"rendered":"Practical Authorship:  Should I write a Case Report?"},"content":{"rendered":"<p><a href=\"https:\/\/blogs.bmj.com\/adc\/files\/2015\/05\/Quill_pen.png\"><img loading=\"lazy\" decoding=\"async\" class=\"alignleft size-thumbnail wp-image-1092\" src=\"https:\/\/blogs.bmj.com\/adc\/files\/2015\/05\/Quill_pen-150x150.png\" alt=\"Quill_pen\" width=\"150\" height=\"150\" \/><\/a>Should <a href=\"https:\/\/blogs.bmj.com\/adc\/2014\/10\/02\/publication-bias\/\">you write a case report<\/a>?<\/p>\n<p>There is a short answer here, and there is a long answer. \u00a0I will give you the short answer, but only if you promise to read the rest. \u00a0The short answer is \u201cno\u201d*. \u00a0The longer answer is&#8230;<\/p>\n<p><!--more--><\/p>\n<p>&#8230;that you really ought to think about what you\u2019re writing when writing a case report. \u00a0My experience is of people coming to me and saying \u201cI&#8217;ve written this case report, but I can\u2019t get it accepted anywhere &#8211; could you tell me why?\u201d \u00a0I look, and very often &#8211; and slightly less bluntly than I&#8217;m going to put here, I have to tell them that:<\/p>\n<ul>\n<li>It\u2019s boring<\/li>\n<li>It\u2019s relevant to very few people<\/li>\n<li>This is often because it is a rare variant of an already rare thing.<\/li>\n<\/ul>\n<p>Now, not wishing to bite the hand that feeds (although I am not paid for blog posts\u2026) I feel duty bound to note that there is an online only model in BMJ publishing, <a href=\"http:\/\/casereports.bmj.com\/\">BMJ case reports<\/a>, which, if you join, allows you to submit reports for peer review. \u00a0Reflect on this: \u00a0A commercial industry has had to set up an entirely different publishing model to deal with the fact that case reports are a struggle to publish. \u00a0There are exceptions to this, but not many.<\/p>\n<p>Please note that I&#8217;m not doubting the value of case reports as a fundamental building block of scientific knowledge. \u00a0Somebody somewhere needed to note that children were dying from a disease that eventually emerged to be understood as cystic fibrosis or Kawasaki disease. \u00a0I\u2019m just saying that perhaps you might consider not writing one. \u00a0Remember that the way that you view things &#8211; that it was absolutely fascinating that this particular complication was associated with a condition that you&#8217;re now much more familiar with. \u00a0But remember the reader. \u00a0The reader will be thinking: \u00a0I&#8217;ve never seen that condition, and so I don&#8217;t want to commit to memory that this rare thing happens. \u00a0And so the editor &#8211; me &#8211; will say &#8220;This may have Merit. \u00a0But we won&#8217;t publish it here&#8230;&#8221;<\/p>\n<p>So, here\u2019s the scenario. \u00a0On the ward round the consultant says \u201cOh, we should write this up as a case report\u201d and looks searchingly at you. \u00a0This is what I think you should do.<\/p>\n<ol>\n<li>There\u2019s no harm in tentatively agreeing. \u00a0Do so, not lying, but not promising either.<\/li>\n<li>Look for that person\u2019s name in <a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\">PubMed<\/a>.<\/li>\n<li>If you can\u2019t find their name in PubMed, and you\u2019re not really excited about the case report in question, then quietly let it drop. \u00a0I&#8217;m really sorry to say this, but the consultant in question probably doesn&#8217;t know what they\u2019re talking about and you could waste a lot of your time.<\/li>\n<li>If they&#8217;ve got lots of publications, especially case reports, work with them, and ignore me &#8211; they know more than I do.<\/li>\n<\/ol>\n<p>Lastly, I was listening to a talk by Professor Alan Craft some years ago, and he quoted Bird\u2019s rule &#8211; and I&#8217;m afraid I don\u2019t recall who Bird was. \u00a0It went: \u00a0if you are adding more than 10% to the world literature, it\u2019s worth trying to publish it. \u00a0So, if you&#8217;ve a case series of two patients, in the context of ten patients in the world so far reported, then it might be worth having a crack at it.<\/p>\n<p>What should you publish instead of a case report? \u00a0That\u2019s soon&#8230;<\/p>\n<p>&#8211; <a href=\"http:\/\/twitter.com\/ian_wac\">@ian_wac<\/a><\/p>\n<p>* Please read the exceptions to &#8216;No&#8217; above.
