I sometimes think altruism is exactly what is missing from the NHS in this day and age, and perhaps it is precisely because it is not valued as it should be. Once what you are doing for people is seen as a contract rather than beyond the call of duty, the relationship changes for the worse. Doctors should be reflecting on this in relation to out of hours.
